单眼先天性白内障摘除术后晶体状态与继发性青光眼的相关性

摘　　要：目的评估单眼先天性白内障患儿术后是否植入人工晶体与继发性青光眼发生的相关性。方法研究纳入自2015年至2019年在我院行白内障摘除联合前段玻璃体切除术的单眼先天性白内障患儿40例,根据患儿是否植入人工晶体分为两组。回顾性分析患儿手术前至手术后3年的眼压、眼前节、房角状态和眼底视盘表现,比较两组患儿继发性青光眼发生率。结果所有患儿中白内障摘除术后继发性青光眼的发生率为12.5%(5/40眼),其中无晶体眼组青光眼的发生率为12% (3/25眼),人工晶体眼组的青光眼发生率为13.3% (2/15眼)。发生青光眼的起始时间一般为术后1年,平均起始时间为19±7.25月。随着随访时间延长,两组青光眼的发生率呈增加趋势。结论单眼先天性白内障术后植入人工晶体对继发性青光眼的发生率并无明显保护作用,且随着时间延长青光眼的发生率呈上升趋势。Objective To evaluate the correlation between intraocular lens implantation and secondary glaucoma in children with monocular congenital cataract.Methods 40 children with monocular congenital cataract who underwent cataract extraction combined with anterior vitrectomy in one hospital from 2015 to 2019 were included.All patients were divided into two groups according to intraocular lens implanted or not.The intraocular pressure,anterior segment,angle state and fundus of patients were analyzed retrospectively.The secondary glaucoma between the two groups and its incidence were compared.Results The total incidence of glaucoma after cataract extraction was 12.5%(5/40 eyes).And the incidence in aphakia group and intraocular lens group was 12%(3/25 eyes) and 13.3%(2/15 eyes) respectively.The average onset time of secondary glaucoma is 19±7.25 months.With the extension of follow-up,the incidence of glaucoma increased in both groups.Conclusion Implantation of intraocular lens after monocular congenital cataract surgery has no obvious protective effect on the incidence of secondary glaucoma,and the incidence of glaucoma tends to increase with time.
