托吡酯联合卡马西平治疗儿童癫痫70例临床观察

摘　　要：目的探讨托吡酯联合卡马西平对儿童癫痫的治疗效果。方法抽取2018年1月至2021年1月濮阳市第五人民医院收治的癫痫患儿70例,随机分为观察组和对照组,每组35例。对照组采用托吡酯单药治疗,观察组采用托吡酯联合卡马西平治疗,比较两组患者疗效、临床症状缓解时间和住院时间、癫痫月发作次数和癫痫发作次数减少率以及不良反应、神经胶质纤维酸性蛋白(GFAP)及血清S-100β蛋白(S-100β)水平。结果观察组临床疗效为94.29%,优于对照组的77.14%(P<0.05);观察组临床症状缓解时间、住院时间和癫痫月发作次数低于对照组(P<0.05),观察组患者癫痫发作次数减少率高于对照组(P<0.05);观察组和对照组患者不良反应发生率分别为25.71%和17.14%,两组比较P>0.05。治疗前,两组患者GFAP、S-100β水平比较,P>0.05;治疗后,两组患者上述指标水平均有降低,且观察组下降幅度大于对照组(P<0.05)。结论采用托吡酯联合卡马西平治疗儿童癫痫疗效良好,可根据临床实际情况进行联合应用。Objective To investigate the therapeutic effect of topiramate combined with carbamazepine on childhood epilepsy.Methods A total of 70 children with epilepsy admitted to Fifth People’s Hospital of puyang the between January 2018 and January 2021 were selected as the research subjects,and were randomly divided into observation group and control group,with 35 cases in each group.The control group was treated with topiramate monotherapy and the observation group was given topiramate combined with carbamazepin.The clinical efficacy,relief time of clinical symptoms,hospital stay,monthly frequency of epileptic seizures,frequency reduction rate of epileptic seizures,adverse reactions and levels of glial fibrillary acidic protein(GFAP)and serum S-100βprotein(S-100β)were compared between the two groups.Results The clinical efficacy of the observation group was 94.29%,which was better than 77.14%of the control group(P<0.05).The clinical symptom relief time,hospital stay and monthly seizure frequency of the observation group were lower than those of the control group(P<0.05),the reduction rate of epileptic seizures in the observation group was higher than that in the control group(P<0.05).The incidence of adverse reactions in the observation group and the control group were 25.71%and 17.14%,respectively,which was compared between the two groups,P>0.05.Before treatment,the levels of GFAP and S-100βbetween the two groups were compared,P>0.05.After treatment,the levels of the above indexes in the two groups decreased,and the decrease range in the observation group was greater than that in the control group(P<0.05).Conclusions Topiramate combined with carbamazepine is effective in the treatment of childhood epilepsy,which can be combined according to the actual clinical situation.
