无托槽隐性矫治技术治疗牙周病致前牙移位患者的效果及其对机体炎性细胞因子水平的影响

摘　　要：目的:探究无托槽隐性矫治技术治疗牙周病致前牙移位患者的效果及其对机体炎性细胞因子水平的影响。方法:选取2016年7月-2020年10月福建省立医院收治的50例牙周病致前牙移位患者,采取随机数字表方法均分为研究组及对照组,各25例。两组均给予基础治疗后,对照组应用传统固定矫正器治疗,研究组应用无托槽隐形矫治器治疗,比较两组临床疗效、治疗前后牙周指数[牙龈指数(GI)、菌斑指数(PLI)和龈沟出血指数(SBI)]及炎性细胞因子水平。结果:研究组、对照组总有效率分别为96.00%(24/25)、76.00%(19/25),研究组总有效率高于对照组(χ;=4.15,P=0.0416);治疗前两组GI、PLI、SBI比较差异均无统计学意义(P>0.05),治疗后两组GI、PLI、SBI均有一定下降,且研究组GI、PLI、SBI均明显低于对照组(t=9.875、3.803、9.303,P=0.000);治疗前两组白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)和白细胞介素-6(IL-6)的水平比较差异均无统计学意义(P>0.05),治疗后两组IL-1β、TNF-α、CRP、IL-6水平均有所下降,且研究组IL-1β、TNF-α、CRP、IL-6水平均明显低于对照组(t=4.812、9.177、3.771、4.126,P=0.000)。结论:在牙周病致前牙移位患者的治疗中应用无托槽隐形矫治技术可以更有效地促进患者牙周情况和炎症反应的改善,与传统固定矫正相比,治疗效果更为理想。Objective:To explore the effect of recessive orthodontic technique without bracket in the treatment of anterior teeth displacement caused by periodontal disease and its effect on the level of body inflammatory cytokines.Method:A total of 50 patients with anterior tooth displacement caused by periodontal disease admitted to Fujian Provincial Hospital from July 2016 to October 2020 were selected and equally divided into study group and control group by random number table method,with 25 cases in each group.After the basic treatment in the two groups,the control group was treated with traditional fixed orthodontic device treatment,and the study group was treated with recessive orthodontic device without bracket treatment.The clinical efficacy,periodontal index[gingival index(GI),plaque index(PLI)and gingival sulcus bleeding index(SBI)]and inflammatory cytokines levels before and after treatment were compared between the two groups.Result:The total effective rate of study group and control group was 96.00%(24/25)and 76.00%(19/25)respectively.The total effective rate of study group was higher than that of control group(χ;=4.15,P=0.0416);before treatment,there were no significant differences in GI,PLI and SBI between the two groups(P>0.05).After treatment,GI,PLI and SBI of the two groups were decreased to some extent,and GI,PLI and SBI of the study group were significantly lower than those of the control group(t=9.875,3.803,9.303,P=0.000).There were no significant differences in the levels of interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and interleukin-6(IL-6)between the two groups before treatment(P>0.05),after treatment,the levels of IL-1β,TNF-α,CRP and IL-6 in the two groups decreased,the levels of IL-1β,TNF-α,CRP and IL-6 in the study group were significantly lower than those in the control group(t=4.812,9.177,3.771,4.126,P=0.000).Conclusion:In the treatment of patients with anterior tooth displacement caused by periodontal disease,the application of recessive orthodontic tec
