新入院精神分裂症患者心肌酶CK、CKMB、LDH、AST的临床检验效果探究

摘　　要：目的:探讨新入院精神分裂症患者心肌酶指标肌酸磷酸激酶(CK)、肌酸激酶同工酶(CKMB)、乳酸脱氢酶(LDH)、天门冬氨酸氨基转移酶(AST)的临床检验效果。方法:选择荆州市复员退伍军人精神病医院2020年10月-2021年9月收治的新入院精神疾病患者248例进行研究,包括精神分裂症、抑郁症、焦虑症、神经症,各62例,并将同期接待的健康体检者50例作为对照。所有纳入对象均检测血清心肌酶指标,包括CK、CKMB、LDH、AST,比较各组检验结果,同时对精神分裂症患者治疗前后进行比较。结果:精神分裂症患者血清心肌酶指标CK、CKMB、LDH、AST均明显高于其他精神疾病与对照组,差异有统计学意义(P<0.05),而抑郁症、焦虑症、神经症与对照组之间对比,差异无统计学意义(P>0.05);精神分裂症患者治疗2周、4周、8周时血清心肌酶指标均低于治疗前,差异有统计学意义(P<0.05),且随着治疗时间延长,呈现明显下降趋势。结论:相比其他精神疾病而言,精神分裂症患者有明显血清心肌酶指标升高的现象,但随着临床治疗,心肌酶指标持续降低,直到恢复正常水平,为此精神科加强血清心肌酶指标检测,可为临床诊断、鉴别及评估精神分裂症提供可靠的依据。Objective:To explore the clinical test effect of myocardial enzyme indexes CK,CKMB,LDH and AST in newly hospitalized schizophrenic patients.Methods:248 newly admitted patients with mental diseases,including 62 cases of schizophrenia,depression,anxiety and neurosis,were selected from Jingzhou demobilized veterans psychiatric hospital from October 2020 to September 2021,and 50 healthy physical examiners received in the same period were used as the control group.All subjects were tested for serum myocardial enzymes,including CK,CKMB,LDH and AST.The test results of each group were compared,and the patients with schizophrenia were compared before and after treatment.Results:The indexes of serum myocardial enzymes CK,CKMB,LDH and AST in schizophrenic patients were significantly higher than those in other mental diseases and the control group(P<0.05),anxiety and neurosis and the control group(P>0.05);The indexes of serum myocardial enzymes in schizophrenic patients at 2,4 and 8 weeks of treatment were lower than those before treatment(P<0.05),and showed a significant downward trend with the extension of treatment time.Conclusion:Compared with other mental diseases,schizophrenic patients have obvious increase of serum myocardial enzyme index,but with clinical treatment,it can continue to decrease until it returns to normal level.Therefore,strengthening the detection of serum myocardial enzyme index in psychiatric department can provide a reliable basis for clinical diagnosis,differentiation and evaluation of schizophrenia.
