摘　　要：目的探讨儿童单核细胞增多性李斯特菌脑膜炎临床特点、发病机制及诊疗策略。方法回顾性分析1例单核细胞增多性李斯特菌脑膜炎患儿的临床资料,并结合文献复习。结果6岁10个月男性患儿,临床表现为发热、腹泻、呕吐、意识障碍,脑脊液检测提示压力增高,细胞数增多,单核细胞为主,蛋白升高,脑脊液培养结果显示,单核细胞增多性李斯特菌生长,临床诊断单核细胞增多性李斯特菌脑膜炎。结论单核细胞增多性李斯特菌脑膜炎临床症状、脑脊液检测结果不典型,病情多发展迅速,并发症多,早期针对性联合用药可能会改善预后。Objective To investigate the clinical characteristics,pathogenesis and treatment strategy of Listeria monocytogenes meningitis in children.Methods The clinical data of a child with Listeria monocytogenes meningitis were retrospectively analyzed and the literature was reviewed.Results The patients was a male child of 6-year and 10 month old,clinicalsymptoms presented as fever,diarrhea,vomiting and disturbance of consciousness.Cerebrospinal fluid test indicated increasing pressure,and cell numberincreased,and monocytes was the dominant cell,the concentration of protein increased.Cerebrospinal fluid culture showed the growth of Listeria monocytogenes.Clinical diagnosis of Listeria monocytogenes meningitis was made.Conclusions The clinical symptoms and cerebrospinal fluid test results of Listeria monocytogenes meningitis are not typical,and the disease develops rapidly with many complications.Early targeted combination therapy may improve the prognosis.
