经颅多普勒超声技术联合屏气试验评估脑梗死患者脑血管储备功能的临床价值

摘　　要：目的研究经颅多普勒超声技术联合屏气试验评估脑梗死患者脑血管储备功能的临床价值。方法将2019年11月至2020年12月于我院住院40例脑梗死患者设为脑梗组,另外选取同时间段至我院体检的40例健康人群,设为健康组。对全部受检者进行经颅多普勒彩色超声及屏气试验检查,并进行二诊。比较脑梗组与健康组受检者的初诊、二诊颅脑中动脉平均血流速度、脑血流指标。观察在二诊时不同障碍程度患者的脑血流指标。结果屏气前、后,脑梗组的初诊、二诊颅脑中动脉平均血流速度均低于健康组(P<0.05);屏气后,两组的初诊、二诊颅脑中动脉平均血流速度均升高,但脑梗组的变化幅度小于健康组(P<0.05)。脑梗组患者初诊、二诊的BHI、RBFV均低于健康组,PI高于健康组(P<0.05);脑梗组患者二诊的BHI、PI、RBFV均低于初诊(P<0.05)。脑梗死患者的功能障碍程度越高,其BHI、RBFV越低,PI越高。结论经颅多普勒超声技术联合屏气试验可较好地评估脑梗死患者的脑血管储备功能,值得应用。Objective To study the clinical value of transcranial Doppler ultrasound combined with breath holding test in evaluating cerebrovascular reserve function in patients with cerebral infarction.Methods A total of 40 patients with cerebral infarction hospitalized in our hospital from November 2019 to December 2020 were set as cerebral infarction group,and 40 healthy people admitted to our hospital for physical examination at the same time were selected as healthy group.All objects were examined by transcranial Doppler ultrasound and breath holding test,and the second diagnosis was carried out.The mean blood flow velocity of middle cerebral artery and cerebral blood flow were compared between the cerebral infarction group and the healthy group.The indexes of cerebral blood flow in patients with different degrees of impairment at the second diagnosis were observed.Results Before and after breath holding,the mean blood flow velocity of middle cerebral artery at the first diagnosis and the second diagnosis in the cerebral infarction group was lower than that in the healthy group(P<0.05);after breath holding,the mean blood flow velocity of middle cerebral artery at the first diagnosis and the second diagnosis in the two groups increased,but the change range of the cerebral infarction group was smaller than that of the healthy group(P<0.05).The BHI and RBFV at the first diagnosis and the second diagnosis in the cerebral infarction group were lower than those in the healthy group,while the PI was higher than that in the healthy group(P<0.05);the BHI,PI and RBFV at the second diagnosis in the cerebral infarction group were lower than those at the first diagnosis(P<0.05).The higher the degree of dysfunction in patients with cerebral infarction,the lower the BHI,RBFV,and the higher the PI.Conclusion Transcranial Doppler ultrasound combined with breath holding test can better evaluate the cerebrovascular reserve function of patients with cerebral infarction,which is worthy of application.
