丁苯酞联合长春西汀治疗椎动脉优势型后循环缺血性眩晕的效果

摘　　要：目的探讨丁苯酞联合长春西汀治疗椎动脉优势型后循环缺血性眩晕的效果。方法选择2018年1月-2020年11月本院神经内科收治的194例椎动脉优势型后循环缺血性眩晕患者为研究对象,随机分为单药治疗组(97例)和联合治疗组(97例)。单药治疗组采用长春西汀进行治疗,联合治疗组在单药治疗组的基础上采用丁苯酞进行治疗,两组均治疗2周。比较两组疗效、动脉血流速度、血液流变学指标和血脂水平。结果治疗后,联合治疗组总有效率为92.78%,显著高于单药治疗组的82.47%,差异有统计学意义(P<0.05);与治疗前相比,治疗后两组患者右侧大脑后动脉(RPCA)、右侧椎动脉(RVA)、左侧大脑后动脉(LPCA)平均血流速度及血清HDL-C水平升高,且联合治疗组高于单药治疗组,差异有统计学意义(P<0.05);与治疗前相比,治疗后两组血液黏度、全血低切黏度、全血高切黏度及血清总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平降低,且联合治疗组低于单药治疗组,差异均有统计学意义(P<0.05)。结论丁苯酞联合长春西汀治疗椎动脉优势型后循环缺血性眩晕可有效改善患者动脉血流速度、血液流变学指标及血脂代谢,具有良好的治疗效果。Objective To analyze the effect of butylphthalide combined with vinpocetine in the treatment of vertebral artery predominant posterior circulation ischemic vertigo.Methods From January 2018 to November 2020,194 patients with vertebral artery dominance posterior circulation ischemic vertigo those were admitted to neurology department of our hospital were enrolled as study objects,and they were randomly divided into mono-therapy group(97 cases)and combination therapy group(97 cases).The single-agent treatment group was treated with vinpocetine,and the combined treatment group was treated with butylphthalide on the basis of the single-agent treatment group.Both groups were treated for 2 weeks.The efficacy,arterial blood flow velocity,hemorheology indexes and blood lipid indexes were compared between the two groups.Results After treatment,the total effective rate of the combined treatment group was 92.78%,which was significantly higher than that of the single-agent treatment group(82.47%),and the difference was statistically significant(P<0.05).Compared with those before treatment,the average blood flow velocity of right posterior cerebral artery(RPCA),right vertebral artery(RVA),left posterior cerebral artery(LPCA)and the levels of serum HDL-C in the two groups increased after treatment,and the combined treatment group was higher than the single-agent treatment group,the differences were statistically significant(P<0.05).Compared with those before treatment,blood viscosity,whole blood low-shear viscosity,whole blood high-shear viscosity and the levels of serum TC,LDL-C decreased in the two groups after treatment,and the combined treatment group was lower than the single-agent treatment group,the differences were statistically significant(P<0.05).Conclusions Butylphthalide combined with vinpocetine could effectively improve arterial blood flow velocity,hemorheological indexes and blood lipid metabolism in patients with vertebral artery dominant posterior circulation ischemic vertigo,and has a good therapeutic effect.
