摘　　要：目的探讨原发性肺上皮-肌上皮癌(EMC)的病理组织学特点、诊断及鉴别诊断。方法通过组织学、免疫组织化学染色技术对6例原发性肺EMC的标本进行组织学观察及免疫标记,对其组织形态和免疫组化特点等进行综合分析。结果 EMC由内层腺上皮及外层肌上皮细胞构成,呈双套管结构。腺上皮成分CK,EMA呈阳性表达,肌上皮成分Calponin, P63,CK5呈阳性表达。结论 EMC罕见,几乎均位于大支气管内,临床症状无特异性,确诊依赖于病理组织学形态及免疫组织化学标记检查,接受外科手术治疗为主的综合治疗后,总体预后好于其他肺部恶性肿瘤,但也可复发或转移。Objective To investigate the pathological characteristics and differential diagnosis of epithelial-myoepithelial carcinoma(EMC) of the bronchial.Methods Six cases of epithelial-myoepithelial tumor of the bronchus was studied by light microscope and immunohistochemistry.And to analyze the morphological and immunohistochemical features.Results The tumor demonstrated a double-layering glandular structures consisted of inner glandular epithelium layer and outer myoepithelial cells layer.Immunohistochemical staining showed that the adenoid cells were positive for CK,EMA and myoepithelial cells were positive for Calponin, P63,CK5.Conclusion The primary EMC in the bronchial is a very rare tumor.Almost all are located in the large bronchus.The diagnosis is based on histopathological characteristics and immunohistochemical staining.After comprehensive treatment based on surgery, the prognosis is better than other malignant lung cancers.But there will also be relapse and metastasis.
