摘  要:目的分析载瘤动脉的曲率及血流动力学对虹吸段动脉瘤的影响。方法选取牡丹江医学院附属红旗医院2017年1月至2021年5月诊断为单侧虹吸段动脉瘤患者的CT血管造影(CTA)图像45例,分成患侧的载瘤动脉组(n=45)与对侧正常颈内动脉的对照组(n=45)。利用MIMICS软件测量载瘤动脉组与对照组的曲率,分别记为K_(1)和K_(2)。利用ANSYS软件分析时间平均壁面切应力(TAWSS)、时间平均壁面切应力梯度(TAWSSG)及震荡剪切指数(OSI)等血流动力学参数。结果K 1平均值为(0.25±0.05)/mm,K_(2)平均值为(0.19±0.05)/mm,差异有统计学意义(t=12.259,P<0.01)。载瘤动脉组血管内侧壁较对照组有更高的TAWSS、TAWSSG及OSI。结论颈内动脉虹吸段的曲率越大,越易促进动脉瘤的形成,同时大曲率载瘤动脉的内侧壁TAWSS、TAWSSG及OSI也明显升高,为诊断虹吸段动脉瘤提供重要的参考依据。Objective To analyze the impact of curvature and hemodynamics of the parent arteries on the aneurysm of the siphon segment.Methods A total of 45 CT angiography(CTA)images of patients diagnosed with aneurysm on the unilateral siphon segment from January 2017 to May 2021 at Hongqi Hospital Affiliated to Mudanjiang Medical University were selected and divided into the group of parent arteries on the affected side(n=45)and the control group of normal internal carotid artery on the contralateral side(n=45).The curvatures of the parent arteries group and the control group were measured using Materialise’s Interactive Medical Image Control System(MIMICS)Software and were recorded as K_(1)and K_(2),respectively.Hemodynamic parameters such as time-averaged wall shear stress magnitude(TAWSS),time average wall shear stress grade(TAWSSG)and oscillatory shear index(OSI)were analyzed using ANSYS software.Results The mean value of K_(1)was(0.25±0.05)/mm and the mean value of K_(2)was(0.19±0.05)/mm,with statistically significant differences(t=12.259,P<0.01).The medial wall of the vessels in the parent arteries group had higher TAWSS,TAWSSG and OSI than those in the control group.Conclusion The larger the curvature of the siphon segment of the internal carotid artery,the more likely it is to promote the formation of aneurysm,and also the TAWSS,TAWSSG and OSI of the medial wall were significantly increasedin the parent arteries with large curvature,which provides an important reference basis for the diagnosis o f aneurysm of the siphon segment.
