胆总管切开取石术联合胆肠吻合内引流术治疗肝内胆管结石的效果分析

摘　　要：目的:研究用胆总管切开取石术联合胆肠吻合内引流术治疗肝内胆管结石的效果。方法:选择2018年4月至2021年4月期间广东省雷州市人民医院收治的124例肝内胆管结石患者作为研究对象。按照治疗方法的不同将其分为比较组和试验组,每组各有患者62例。对比较组患者进行胆总管切开取石术联合T管引流术,对试验组患者进行胆总管切开取石术联合胆肠吻合内引流术,然后比较两组患者的疗效及各项临床指标。结果:试验组患者治疗的优良率高于比较组患者,差异有统计学意义(P<0.05)。术后,试验组患者并发症的发生率、结石的残留率和病情的复发率均低于比较组患者,差异有统计学意义(P<0.05)。结论:用胆总管切开取石术联合胆肠吻合内引流术对肝内胆管结石患者进行治疗的效果显著,能有效减轻其临床症状,且其术后并发症的发生率、结石的残留率和病情的复发率均较低,治疗的安全性较高。Objective:To study the effect of choledocholithotomy combined with choledochoenterostomy internal drainage in the treatment of intrahepatic bile duct calculi.Methods:A total of 124 patients with intrahepatic cholangiolithiasis admitted to Leizhou People’s Hospital of Guangdong Province from April 2018 to April 2021 were selected as the subjects.According to the different treatment methods,they were divided into comparison group and experimental group,with 62 patients in each group.Patients in the comparison group were treated with choledocholithotomy combined with T tube drainage,and patients in the experimental group were treated with choledocholithotomy combined with bilioenterostomy internal drainage,and then the efficacy and clinical indicators of the two groups were compared.Results:The rate of excellent and good treatment in experimental group was higher than that in comparison group,the difference was statistically significant(P<0.05).After surgery,the incidence of complications,residual stone rate and disease recurrence rate in the experimental group were lower than those in the comparison group,with statistical significance(P<0.05).Conclusion:Choledocholithotomy combined with choledochoenterostomy drainage has a significant effect on the treatment of patients with intrahepatic bile duct calculi,which can effectively reduce the clinical symptoms,and the incidence of postoperative complications,stone residual rate and disease recurrence rate are low,and the safety of treatment is high.
