摘　　要：目的探讨内皮PAS结构域包含蛋白-1(EPAS-1)在透明细胞性肾细胞癌中的表达特征,分析其与细胞增殖的关系及其临床意义。方法收集在我院确诊为透明细胞性肾细胞癌的患者共78例,留取术后肿瘤组织作为观察组,留取距肿瘤标本肉眼可见的边缘>3 cm的正常肾组织作为对照组,应用免疫组化法检测两组中EPAS-1及观察组中增殖细胞核抗原(PCNA)的表达,分析其临床意义;选择人肾透明细胞癌786-0细胞系和人肾皮质近曲小管上皮HK-2细胞系,应用Western Blot方法检测EPAS-1蛋白的表达,实时荧光定量PCR法(RT-PCR)检测EPAS-1 mRNA的表达。选择人肾透明细胞癌786-0细胞系,建立小干扰RNA EPAS-1(siRNA EPAS-1组),并设立786-0细胞系的空白对照组(NC组),Western Blot检测PCNA蛋白,应用RT-PCR法检测PCNA mRNA。结果观察组中EPAS-1表达的阳性率明显高于对照组(P<0.05),EPAS-1的阳性率在不同Fuhrman分级、肿瘤最大径及PCNA增殖指数的表达中差异均有统计学意义(P<0.05),而在有无肾窦累犯、不同性别及年龄分组的表达中差异无统计学意义(P>0.05)。EPAS-1的表达与生存时间有关(P<0.05)。人肾透明细胞癌786-0细胞系中EPAS-1蛋白和mRNA的表达明显高于人肾皮质近曲小管上皮HK-2细胞系(P<0.05)。siRNA EPAS-1组中PCNA蛋白和mRNA的表达明显低于NC组(P<0.05)。结论EPAS-1在透明细胞性肾细胞癌中表达明显升高,在不同临床病理特征中的表达有差异。EPAS-1可能对促进肿瘤细胞增殖有一定作用。检测EPAS-1的表达可能与肿瘤的预后有关。Objective To observe the expression of EPAS-1 in clear cell renal cell carcinoma,analyze its relationship with cell proliferation,investigate clinical significance.Methods 78 patients with clear cell renal cell carcinoma were collected.The tumor tissue was taken as the observation group,and the normal renal tissue(>3cm from the tumor margin)was taken as the control group.The expression of EPAS-1 of two groups and proliferating cell nuclear antigen(PCNA)of observation group were detected by immunohistochemistry method.Human renal clear cell carcinoma 786-0 cell line and human renal cortex proximal convoluted tubule epithelial HK-2 cell line were selected.The expression of EPAS-1 protein was detected by Western blot,the expression of EPAS-1 mRNA was detected by real-time fluorescence quantitative PCR(qRT-PCR).Human renal clear cell carcinoma 786-0 cell line was selected to establish small interfering RNA EPAS-1(siRNA EPAS-1 group)and a blank control group(NC group).PCNA protein was detected by Western blot and PCNA mRNA was detected by RT-PCR.Results Positive rate of EPAS-1 was significantly higher in clear cell renal cell carcinoma than that in the control group.The positive rate of EPAS-1 was significantly different in Fuhrman grade,diameter and PCNA proliferation index,but was not significantly different in renal sinus involvement,gender and age.The expression of EPAS-1 was related to survival time.The expression of EPAS-1 protein and mRNA was significantly higher in 786-0 cell line than that in HK-2 cell line.The expression of PCNA protein and mRNA was significantly lower in siRNA EPAS-1 group than that in NC group.Conclusion Expression of EPAS-1 is significantly increased in clear cell renal cell carcinoma.Expression of EPAS-1 is different in different clinicopathological features.EPAS-1 may play a role in promoting cell proliferation.The expression of EPAS-1 may be related to the prognosis of tumor.
