免疫组织化学检测在乳腺癌病理诊断中的应用效果

摘　　要：目的探讨免疫组织化学检测在乳腺癌病理诊断中的应用效果。方法180例乳腺癌患者作为研究对象,分为雌激素受体(ER)(+)孕激素受体(PR)(+)组和ER(-)PR(-)组,各90例。两组患者术前均接受免疫组织化学检测。比较两组患者的C-erbB-2阳性检出率以及腋窝淋巴结转移率。结果ER(+)PR(+)组患者的C-erbB-2阳性检出率28.9%明显低于ER(-)PR(-)组的75.6%,差异具有统计学意义(P<0.05)。ER(+)PR(+)组患者腋窝淋巴结转移30例(33.3%),未转移60例(66.7%);ER(-)PR(-)组患者腋窝淋巴结转移49例(54.4%),未转移41例(45.6%)。ER(+)PR(+)组患者的腋窝淋巴结转移率低于ER(-)PR(-)组,差异有统计学意义(χ^(2)=8.144,P=0.004<0.05)。结论免疫组织化学检测在乳腺癌病理诊断中的应用效果显著,可用来评估患者的内分泌情况、治疗后的可预见效果及预期目标,也方便医护人员全面了解乳腺癌患者的病情和确诊手术方案,对患者的乳腺癌严重程度、病情改变状态、速度以及淋巴结转移情况进行充分了解,对整个治疗过程有着积极的推动作用,值得在临床医学上广泛推广应用。Objective To discuss the application effect of immunohistochemical testing in pathological diagnosis of breast cancer.Methods A total of 180 breast cancer patients as the research subjects were divided into estrogen receptor(ER)(+)progesterone receptor(PR)(+)group and ER(-)PR(-)group,with 90 cases in each group.Both groups received immunohistochemical testing before surgery.The positive detection rate of C-erbB-2 and axillary lymph node metastasis rate were compared between the two groups.Results The positive detection rate of C-erbB-2 in the ER(+)PR(+)group was 28.9%,which was significantly lower than 75.6%in the ER(-)PR(-)group,and the difference was statistically significant(P<0.05).In the ER(+)PR(+)group,30 patients had axillary lymph node metastasis(33.3%),and 60 patients had no metastasis(66.7%);in the ER(-)PR(-)group,49 cases(54.4%)had axillary lymph node metastasis,and 41 cases(45.6%)had no metastasis.The axillary lymph node metastasis rate of the ER(+)PR(+)group was lower than that of the ER(-)PR(-)group,and the difference was statistically significant(χ^(2)=8.144,P=0.004<0.05).Conclusion Immunohistochemical testing has significant effect in pathological diagnosis of breast cancer,and can be used to assess the endocrine status of patients,the predictable effect of treatment and the expected goal,and also facilitate the medical staff to fully understand the condition of breast cancer patients and confirm the surgical plan,and fully understand the severity of breast cancer,the state of change,the speed and the lymph node metastasis of patients,which has a positive effect on the whole treatment process.It has a positive effect on the whole treatment process,and is worth to be widely promoted and applied in clinical medicine.
