快速康复外科理念对单孔胸腔镜肺癌根治术后恢复的作用分析

摘　　要：目的探讨快速康复外科理念对单孔胸腔镜肺癌根治术后恢复的临床作用。方法284例单孔胸腔镜肺癌根治术患者,依据随机数字表法分为研究组与常规组,各142例。常规组给予常规干预,研究组实施快速康复外科理念。比较两组焦虑自评量表评分、视觉模拟评分法评分、胸腔引流管留置时间、下床时间、住院时间、并发症发生率。结果研究组焦虑自评量表评分(46.45±1.21)分、视觉模拟评分法评分(1.25±0.21)分均显著低于常规组的(59.56±2.45)、(3.46±0.45)分,胸腔引流管留置时间(45.04±2.51)h、下床时间(1.35±0.45)d、住院时间(5.25±1.02)d,均短于常规组的(68.12±3.18)h、(3.52±1.01)d、(7.22±2.18)d,差异有统计学意义(P<0.05)。研究组的并发症发生率为2.82%(4/142)低于常规组的20.42%(29/142),差异有统计学意义(P<0.05)。结论单孔胸腔镜肺癌根治术患者实施快速康复外科理念效果确切,可缓解患者焦虑情绪,减轻疼痛,且可加速术后的恢复,缩短胸腔引流管留置时间,减少并发症,是一种值得推广的干预模式。Objective To discuss the clinical effect of fast-track surgery concept on recovery after single-port thoracoscopic radical resection of lung cancer.Methods A total of 284 patients undergoing singleport thoracoscopic radical resection of lung cancer were divided into research group and conventional group according to random numerical table,with 142 cases in each group.The conventional group received conventional intervention,and the research group received fast-track surgery concept.Both groups were compared in terms of self-rating anxiety scale score,visual analogue scale score,indwelling time of thoracic drainage tube,off-bed time,hospitalization time,and complication rate.Results The self-rating anxiety scale score(46.45±1.21)points and visual analogue scale score(1.25±0.21)points of the research group were significantly lower than(59.56±2.45)and(3.46±0.45)points of the conventional group;the indwelling time of thoracic drainage tube(45.04±2.51)h,off-bed time(1.35±0.45)d,hospitalization time(5.25±1.02)d of the research group were shorter than(68.12±3.18)h,(3.52±1.01)d,(7.22±2.18)d of the conventional group;all the differences were statistically significant(P<0.05).The complication rate 2.82%(4/142)of the research group was lower than 20.42%(29/142)of the conventional group,and the difference was statistically significant(P<0.05).Conclusion Fast-track surgery concept has definite effect on patients undergoing single-port thoracoscopic radical resection of lung cancer,and it can alleviate patients'anxiety and pain,accelerate postoperative recovery,shorten the indwelling time of thoracic drainage tube and reduce complications.It is a nursing model worthy of popularization.
