三孔法经腹膜外入路腹腔镜前列腺癌根治术治疗前列腺癌患者的临床疗效探讨

摘　　要：目的探讨三孔法经腹膜外入路腹腔镜前列腺癌根治术(ELRP)治疗前列腺癌患者的临床疗效。方法64例前列腺癌患者,依据随机数字表法分为参考组与试验组,各32例。试验组采用三孔法ELRP治疗,参考组采用四孔法ELRP治疗。对比两组临床指标、术后Gleason评分及切缘阳性发生率、术后尿失禁及生化复发情况。结果试验组术中出血量(105.38±17.66)ml少于参考组的(135.49±19.58)ml,手术时间(118.67±20.31)min、术后引流管留置时间(3.06±1.17)d、术后导尿管留置时间(5.84±1.12)d及出院时间(6.93±1.22)d均短于参考组的(146.37±21.78)min、(4.98±1.56)d、(8.21±1.43)d及(8.65±1.10)d,差异均有统计学意义(P<0.05)。试验组Gleason评分及切缘阳性发生率均略优于参考组,但差异均无统计学意义(P>0.05)。两组术后30 d尿失禁发生率、术后180 d尿失禁发生率、生化复发率对比,差异均无统计学意义(P>0.05)。结论与四孔法ELRP相比,采用三孔法ELRP治疗前列腺癌可明显缩短手术时长,减少出血量,有利于促进机体功能恢复。Objective To discuss the cclinical efficacy of three-port approach for extraperitoneal laparoscopic radical prostatectomy(ELRP)in the treatment of prostate cancer patients.Methods According to random numerical table,64 patients with prostate cancer were divided into reference group and test group,with 32 cases in each group.The test group was treated with three-port ELRP,and the reference group was treated with four-port ELRP.The clinical indicators,postoperative Gleason score and incidence of positive margins,postoperative urinary incontinence and biochemical recurrence were compared between the two groups.Results The intraoperative blood loss(105.38±17.66)ml of the test group was less than(135.49±19.58)ml of the reference group;the operation time(118.67±20.31)min,postoperative drainage tube indwelling time(3.06±1.17)d,postoperative catheter indwelling time(5.84±1.12)d,and discharge time(6.93±1.22)d of the test group were shorter than(146.37±21.78)min,(4.98±1.56)d,(8.21±1.43)d and(8.65±1.10)d of the reference group;all the differences were statistically significant(P<0.05).The Gleason score and the incidence of positive margins in the test group were slightly better than those in the reference group,but the differences were not statistically significant(P>0.05).There was no statistically significant difference in the incidence of urinary incontinence at 30 d postoperatively,the incidence of urinary incontinence at 180 d postoperatively,and biochemical recurrence rate between the two groups(P>0.05).Conclusion Compared with the four-port ELRP,the three-port ELRP can significantly shorten the operation time,reduce the blood loss,and promote the recovery of body function.
