不同手术方式治疗眼外伤继发青光眼的临床效果对比

摘　　要：目的分析不同手术方式对于眼外伤继发青光眼的临床治疗效果。方法90例眼外伤继发青光眼患者,采用随机数字表法分为参照组(45例,47眼)和研究组(45例,48眼)。参照组患者采取周边虹膜切除术治疗,研究组患者采取小梁切除术治疗。比较两组患者手术成功率、并发症发生情况、手术前后眼压及裸眼视力。结果研究组手术成功率为93.75%,失败率为6.25%;参照组手术成功率为78.72%,失败率为21.28%;研究组手术成功率高于参照组,差异具有统计学意义(P<0.05)。研究组患者并发症发生率为2.08%,低于参照组的6.38%,但比较差异无统计学意义(P>0.05)。术前,研究组患者眼压为(61.16±4.98)mm Hg(1 mm Hg=0.133 kPa)、裸眼视力为(0.02±0.04),与参照组的(60.92±4.87)mm Hg、(0.02±0.03)比较,差异无统计学意义(P>0.05)。术后,研究组患者眼压(21.16±4.85)mm Hg低于参照组的(39.12±4.33)mm Hg,裸眼视力(0.17±0.04)高于参照组的(0.07±0.03),差异具有统计学意义(P<0.05)。结论小梁切除术能够提升眼外伤继发青光眼治疗成功率,降低并发症发生率,改善眼压及裸眼视力。Objective To analyze the clinical effect of different surgical methods for the treatment of glaucoma secondary to ocular trauma.Methods A total of 90 patients with glaucoma secondary to ocular trauma were divided into reference group(45 cases,47 eyes)and research group(45 cases,48 eyes)according to random numerical table.Patients in the reference group were treated with peripheral irisectomy,and patients in the research group were treated with trabeculectomy.The surgical success rate,complications,intraocular pressure and naked visual acuity before and after surgery were compared between the two groups.Results The success rate of surgery in the research group was 93.75%,and the failure rate was 6.25%;the success rate of surgery in the reference group was 78.72%,and the failure rate was 21.28%;the success rate of surgery in the research group was higher than that in the reference group,and the difference was statistically significant(P<0.05).The complication rate of the research group was 2.08%,which was lower than 6.38%of the reference group,but the difference was not statistically significant(P>0.05).Before surgery,the intraocular pressure in the research group was(61.16±4.98)mm Hg(1 mm Hg=0.133 kPa),and the naked visual acuity was(0.02±0.04),which were not statistically significant compared with(60.92±4.87)mm Hg and(0.02±0.03)in the reference group(P>0.05).After surgery,the intraocular pressure(21.16±4.85)mm Hg of the research group was lower than(39.12±4.33)mm Hg of the reference group,and the naked visual acuity(0.17±0.04)was higher than(0.07±0.03)of the reference group.All the differences were statistically significant(P<0.05).Conclusion Trabeculectomy can increase the success rate of glaucoma secondary to ocular trauma,reduce the complication rates,and improve intraocular pressure and naked visual acuity.
