清解化攻方辅助治疗瘀毒互结型高脂血症性急性胰腺炎的临床观察

摘　　要：目的观察自拟中药清解化攻方辅助治疗瘀毒互结型高脂血症性急性胰腺炎(HLAP)的临床疗效。方法将65例瘀毒互结型HLAP患者随机分为对照组(32例)和观察组(33例)。对照组采用西医常规治疗,观察组在西医常规治疗的基础上口服清解化攻方,两组疗程均7 d。比较两组临床疗效及治疗前后血清白细胞计数、C反应蛋白(CRP)、降钙素原、肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)、三酰甘油、淀粉酶、脂肪酶水平及治疗后的并发症发生率。结果观察组总有效率(90.6%)高于对照组(69.7%),并发症发生率低于对照组(均P<0.05);治疗后,两组患者血清白细胞计数、CRP、降钙素原、TNF-α及IL-6、三酰甘油、淀粉酶、脂肪酶水平均较治疗前降低,且观察组上述各指标水平低于对照组(均P<0.05)。结论在西医治疗基础上口服清解化攻方治疗瘀毒互结型HLAP临床疗效好,能有效地降低患者血清炎症指标、血脂水平,促进胰腺功能修复,减少并发症发生率,值得临床推广。Objective To observe the clinical efficacy of adjuvant therapy with self-designed traditional Chinese drug,Qing Jie Hua Gong Fang,for hyperlipidemic acute pancreatitis(HLAP)with syndrome of intermingling blood stasis and toxin.Methods Sixty-five HLAP patients with syndrome of intermingling blood stasis and toxin were randomly divided into control group(n=32)and observation group(n=33).The control group was treated with routine western medicine,and the observation group was administrated Qing Jie Hua Gong Fang orally on the basis of routine western medicine therapy.The course of treatment was seven days in both groups.A comparison between the two groups was performed with respect to clinical efficacy,as well as serum levels of WBC count,C-reactive protein(CRP),procalcitonin,tumor necrosis factorα(TNF-α),interleukin 6(IL-6),TG,amylase,and lipase before and after the treatment,and the incidence rate of complications after the treatment.Results The total effective rate of the observation group(90.6%)was higher than that of the control group(69.7%),and the incidence rate of complications was lower in the observation group than in the control group(all P<0.05).After the treatment,serum levels of WBC count,CRP,procalcitonin,TNF-α,IL-6,TG,amylase,and lipase in both groups were decreased as compared with those before the treatment,and the levels of the aforementioned indicators in the observation group were lower than those in the control group(all P<0.05).Conclusion Oral administration of Qing Jie Hua Gong Fang on the basis of western medicine treatment exerts favorable clinical efficacy for HLAP with syndrome of intermingling blood stasis and toxin,which can effectively reduce serum inflammation indicators and blood lipid levels,promote the recovery of pancreatic function and diminish the incidence rate of complications in the patients.And it is worthy of clinical promotion.
