摘　　要：目的探索血清淀粉样蛋白(SAA)在肺炎支原体(MP)感染中的变化,为MP感染的早期诊断、疗效判定提供新的临床参考指标。方法选取2020年1~12月在泰州市人民医院儿住院的普通MP肺炎患者45例为病例组,同期在儿保科、儿外科进行体检或择期手术的患者40例作为健康对照组,通过收集病例组和对照组的临床资料、血液检查结果进行统计学分析。结果性别比例、年龄、体质指数(BMI)、白细胞总数、中性粒细胞比例、谷丙转氨酶、尿素氮、SAA/CRP在两组间均无显著差异(P>0.05)。C反应蛋白(CRP)在病例组和对照组分别为(11.75±6.74)mg/L、(3.61±1.62)mg/L,P<0.001;SAA分别为(82.06±40.09)mg/L、(29.46±16.11)mg/L,P<0.001。恢复期平均SAA(29.08±16.94)mg/L,平均CRP(4.2±1.61)mg/L,较入组时均有明显下降。ROC曲线分析SAA、CRP、SAA/CRP发现,曲线下面积(AUC)分别为0.94、0.93、0.45。结论SAA和CRP在MP感染的早期诊断和治疗后疗效的判定方面均有较好的应用价值,且SAA较CRP具有更好的灵敏度和特异性,值得临床推广使用。Objective To explore the changes of serum amyloid(SAA)in Mycoplasma pneumoniae(MP)infection,and to provide new clinical reference indicators for the early diagnosis and therapeutic effect of MP infection.Methods 45 cases of general MP pneumonia patients who were hospitalized in Taizhou People’s Hospital from January 2020 to December 2020 were selected as the case group,and 40 patients who underwent physical examination or elective surgery in the Department of Pediatrics and Pediatric Surgery during the same period were selected as the healthy control group.The clinical data and blood test results of the case group and the control group were analyzed statistically.Results There were no significant differences in gender ratio,age,body mass index(BMI),total number of white blood cells,neutrophil ratio,alanine aminotransferase,urea nitrogen,SAA/CRP between the two groups.C-reactive protein(CRP)was(11.75±6.74)mg/L and(3.61±1.62)mg/L in the case group and control group,respectively,with P<0.001;SAA was(82.06±40.09)mg/L and(29.46±16.11)mg/L,respectively L,P<0.001.During the recovery period,the average SAA(29.08±16.94)mg/L and the average CRP(4.2±1.61)mg/L were significantly lower than those at the time of enrollment.ROC curve analysis SAA,CRP,SAA/CRP found that the area under the curve(AUC)were 0.94,0.93,0.45,respectively.Conclusion Both SAA and CRP have good application value in the early diagnosis of MP infection and the evaluation of curative effect after treatment.SAA has better sensitivity and specificity than CRP and is worthy of clinical application.
