躯体化症状自评量表在功能性消化道不良诊治中的应用

摘　　要：目的探讨躯体化症状自评量表(Somatic Self-rating Scale,SSS)在功能性消化不良(Functional Dyspepsia,FD)患者中应用的信度和效度,并以评估其临床应用价值,为医护人员提供科学和有效的诊断参考,并为进一步治疗和随访提供依据。方法以整群随机抽样法,选择2019年1~12月就诊于乐山市人民医院的65例功能性消化不良患者作为研究对象,通过问卷法进行躯体化症状自评量表评分,通过对得分进行信度和效度检验、探索因子分析、项目分析、Cronbach’sα系数分析,评估该量表在功能性消化不良患者中的有效性。结果本研究中功能性消化不良患者的躯体化症状自评量表总分为(40.1±9.6)分,被调查者在不同的性别、年龄、文化程度等方面计分差异均不具有统计学差异(P>0.05)。量表中共20项条目,其中包含4个因子,分别为由躯体化症状(S因子)、焦虑症状(A因子)、抑郁症状(D因子)、焦虑抑郁症状(AD因子)总计四个部分组成,各因子得分和量表总得分之间相关系数为0.639~0.814,各分子之间相关系数为0.314~0.591,差异均具有统计学意义(P<0.05),表明各条目在同质性方面与总量表匹配较好。总量表与各因子的内部的一致性信度Cronbach’sα系数分别为0.823、0.810、0.711、0.707、0.622。结论躯体化症状自评量表在评估功能性消化不良患者的负面情绪状态方面有良好的结构效度和内部一致性信度,可以初步反映其心理状况,满足筛查需要。Objective To investigate the reliability and validity of somatic self-rating scale(SSS)in patients with functional dyspepsia(FD),and to evaluate its clinical application value,so as to provide scientific and effective diagnostic reference for medical staff,and provide basis for further treatment and follow-up.Methods By cluster random sampling method,a total of 65 patients with functional dyspepsia who were admitted to Leshan People’s Hospital from January to December 2019 were selected as the study subjects.somatic self-rating scale was scored by questionnaire method.The validity of the scale in patients was evaluated by reliability and validity test,exploration factor analysis,item analysis and Cronbach’sαcoefficient analysis.Results In this study,the total SSS score of the patients with functional dyspepsia was(40.1±9.6),and there was no statistical significance in the scores of patients with different genders,ages and educational levels(P>0.05).20 items in the scale,including four factors,respectively for somatization symptoms factor(S factor),anxiety symptoms(A factor),depressive symptoms(D factor),anxiety,depression factor(AD),each factor score with scale scores between the correlation coefficient 0.639~0.814,the correlation coefficient 0.314~0.591 between molecules,and the differences were statistically significant(P<0.05),suggesting that each item m atched well with the total table in term s of homogeneity.The Cronbach’sαcoefficient of the total table and the internal consistency reliability of each factor were 0.823,0.810,0.711,0.707 and 0.622,respectively.Conclusion The somatic self-rating scale has good structural validity and internal consistency reliability in evaluating the negative emotional state of patients with functional dyspepsia,which can initially reflect their psychological status and meet the needs of screening.
