摘  要:目的检测人乳头瘤病毒(HPV)E6/E7 mRNA、P53及P16在宫颈脱落细胞中的表达,分析其在未能明确意义的不典型鳞状细胞(ASCUS)分层管理中的应用。方法选取2016年4月—2018年10月郑州大学附属肿瘤医院220例宫颈液基薄层细胞学(TCT)检查结果为ASCUS的患者,采用支链DNA技术和细胞涂片免疫组化法检测宫颈脱落细胞中HPV E6/E7 mRNA、P53及P16表达情况。结果宫颈炎、低度鳞状上皮内病变(LSIL)及高度鳞状上皮内病变(HSIL)患者中HPV E6/E7 mRNA、P53及P16阳性率比较差异均有统计学意义(均P<0.05)。HPV E6/E7 mRNA与P53蛋白不相关(r=1.107,P=0.112),HPV E6/E7 mRNA与P16蛋白呈正相关(r=0.363,P=0.000),P53与P16蛋白呈正相关(r=0.477,P=0.000)。结论 HPV E6/E7 mRNA、P53及P16蛋白在ASCUS患者宫颈脱落细胞中的阳性率随宫颈病变程度增加而增加,检测三者可作为ASCUS分流管理的方法。Objective To detect the expressions of human papillomavirus(HPV) E6/E7 mRNA,P53,and P16 in cervical exfoliated cells,analyze the application in atypical squamous cells of undetermined signification(ASCUS) stratification management.Methods From April 2016 to October 2018,220 patients diagnosed as ASCUS based on thinprep cytologic test(TCT) were selected from Affiliated Cancer Hospital of Zhengzhou University,branched DNA technology and cell smear immunohistochemical method were used to detect the expressions of HPV E6/E7 mRNA,P53,and P16 in cervical exfoliated cells.Results There were statistically significant differences in the positive rates of HPV E6/E7 mRNA,P53,and P16 among the patients with cervicitis,low-grade squamous intraepithelial lesion(LSIL),and high-grade squamous intraepithelial lesion(HSIL)(P<0.05),HPV E6/E7 mRNA was not correlated with P53 protein(r=1.107,P=0.112),HPV E6/E7 mRNA was positively correlated with P16 protein(r=0.363,P=0.000),P53 was positively correlated with P16 protein(r=0.477,P=0.000).Conclusion The positive rates of HPV E6/E7 mRNA,P53,and P16 protein in cervical exfoliated cells increase with aggravation of cervical lesion,and the detection of these three indexes can be used as the method of ASCUS stratification management.
