超声乳化术治疗葡萄膜炎继发性青光眼伴发白内障的效果观察

摘　　要：目的观察超声乳化术治疗葡萄膜炎继发性青光眼伴发白内障的临床效果。方法回顾性病例系列研究。分析郑州大学第一附属医院2019年1月至2020年10月超声乳化术治疗葡萄膜炎继发性青光眼伴发白内障47例(63只眼)的临床资料。所有患者均行超声乳化术。术后随访6个月,分析术后眼压、视力、前房深度、局部降眼压药物种类及并发症等。结果术后1 d、1周、1、3及6个月的眼压均低于术前(均P<0.001)。术后6个月:最佳矫正视力为0.40±0.09,优于术前1.18±0.26(t=23.776,P<0.001);虹膜膨隆消失,前房深度为(2.97±0.15)mm,较术前(1.75±0.27)mm明显加深(t=-83.917,P<0.001);平均使用降眼压药物种类为(0.38±1.10)种,较术前(2.33±0.98)种类减少(t=13.741,P<0.001);手术完全成功56只眼(88.89%),条件成功6只眼(9.52%),失败1只眼(1.59%)。结论超声乳化术治疗葡萄膜炎继发性青光眼伴发白内障的效果可靠。Objective To observe the clinical efficacy of phacoemulsification in the treatment for secondary glaucoma with cataract due to uveitis.Methods This was a case series retrospective study.The clinical data of 63 eyes of 47 cases with secondary glaucoma and cataract due to uveitis from Jan.2019 to Oct.2020 in the First Affiliated Hospital of Zhengzhou University were analyzed.All cases were treated with phacoemulsification.The follow-up time was 6 months.The postoperative intraocular pressure,visual acuity,anterior chamber depth,the types of intraocular lowering drugs and complications were analyzed.Results The intraocular pressure at 1 day,1 week,1,3 and 6 months after operation were lower than the before operation(all P<0.001).At 6 months after operation,BCVA was 0.40±0.09,which was higher than the 1.18±0.26 before operation(t=23.776,P<0.001).The iris bulge disappeared,and the anterior chamber depth was(2.97±0.15)mm,which was significantly deeper than the(1.75±0.27)mm before surgery(t=-83.917,P<0.001);the average dosage of intraocular pressure lowering drugs used after operation was(0.38±1.10)kinds,which was less than the(2.33±0.98)kinds before operation(t=13.741,P<0.001);the operation was completely successful in 56 eyes(88.89%),completely successful and conditionally successful in a total of 62 eyes(98.41%),and failed in 1 eye(1.59%).Conclusion Phacoemulsification is effective in treating secondary glaucoma with cataract due to uveitis.
