玻璃体切除术联合空气填充治疗特发性黄斑孔的效果

摘　　要：目的评价玻璃体切除术联合空气填充治疗特发性黄斑孔(IMH)的效果。方法回顾分析2018年10月至2020年10月山东中医药大学附属眼科医院收治的特发性黄斑孔79例(79眼)的临床资料。根据手术方式分成两组:空气组41例,C3F8组38例。观察两组手术效果。结果黄斑孔闭合率:空气组为95.12%(38/41),C3F8组为94.74%(36/48)(χ^(2)=0.140,P>0.05);两组术后视力(logMAR)均提高(P>0.05)。俯卧位时间:空气组为(4.12±0.82)d,短于C3F8组的(10.67±1.01)d(t=-31.015,P<0.001);白内障发生率:空气组为21.95%(9/41),低于C3F8组的47.37%(18/38)(χ^(2)=5.663,P=0.020)。结论玻璃体切除术联合空气填充治疗黄斑孔,孔闭合率高,可以缩短俯卧位时间,降低白内障发生率。Objective To evaluate the clinical efficacy of vitrectomy combined with air tamponade in the treatment of idiopathic macular hole(IMH).Methods The clinical data of 79 eyes of 79 patients with IMH from Oct.2018 to Oct.2020 in Ophthalmic Hospital Affiliated to Shandong University of Traditional Chinese Medicine were analyzed retrospectively.They were divided into two groups based on the surgical method:air group with 41 cases and C3F8 group with 38 cases.The efficacy between two groups were observed.Results The closure rate of macular hole in the air group was 95.12%(38/41),and that of C3F8 group was 94.74%(36/38)(χ^(2)=0.140,P>0.05).Visual acuity after surgery improved in the both groups(P>0.05).The prone position time in the air group was(4.12±0.82)days,which was shorter than the(10.67±1.01)days of the C3F8 group(t=-31.015,P<0.001);the incidence of cataract was 21.9%(9/41)in the air group,which was lower than 47.4%(18/38)in the C3F8 group(χ^(2)=5.663,P=0.020).Conclusion Vitrectomy combined with air tamponade in the treatment of IMH is with a high rate of hole closure,which can reduce the time in the prone position,and reduce the degree of lens opacity.
