伤及眼后节的开放性眼外伤玻璃体切除术效果影响因素分析

摘　　要：目的分析伤及眼后节的开放性眼外伤玻璃体切除术的效果影响因素。方法回顾性病例对照研究。分析周口市中心医院2018年4月至2021年6月行玻璃体切除术治疗伤及眼后节的开放性眼外伤者82例(82眼)的临床资料,根据治疗效果分为两组:12例效果不佳组,70例效果良好组,分析影响手术效果的因素。结果82例中总治愈率为85.36%(70/82),并发症发生率7.32%(6/82)。术后3 d、7 d的最佳矫正视力优于术前(P<0.05)。而视力、手术时机、年龄、钝器伤、眼内异物、出血累及黄斑中心凹情况是影响手术效果的危险因素(P<0.05)。结论伤及眼后节的开放性眼外伤实施玻璃体切除术时需考虑视力、年龄、时机、眼内异物等情况。Objective To analyze the prognostic factors of vitrectomy for open ocular trauma with posterior segment involvement.Methods This was a retrospective case control study.The clinical data of 82 eyes of 82 cases of open ocular trauma with posterior segment involvement from Apr.2018 to Jun.2021 in Zhoukou Central Hospital were analyzed.After vitrectomy,the treatment efficacy was observed.According to the treatment results,they were divided into two groups.Twelve cases with poor efficacy were included in the poor efficacy group,and 70 cases with good efficacy were included in the good efficacy group.The influencing of factors efficacy were analyzed.Results The total cure rate was 85.36%(70/82)and the incidence of complication was 7.32%(6/82).The BCVA at 3 and 7 days after operation was better than that before operation(P<0.05).The visual acuity,operation time,age,blunt injury,intraocular foreign body,bleeding and involvement of macular fovea were the risk factors affecting the efficacy(P<0.05).Conclusion The visual acuity,age,timing and intraocular foreign bodies should be considered in the implementation of vitrectomy for open ocular trauma with posterior segment involvement.
