化痰通腑汤治疗缺血性卒中急性期痰热腑实证患者的临床疗效及对神经功能缺损和生活质量的影响

摘　　要：目的观察化痰通腑汤治疗缺血性卒中急性期痰热腑实证患者的临床疗效及对神经功能缺损和生活质量的影响。方法将98例缺血性卒中急性期痰热腑实证患者按照随机数字表法分为2组。对照组49例予常规西医治疗,治疗组49例在对照组治疗基础上联合化痰通腑汤治疗。2组均治疗10 d。比较2组治疗前后临床症状评分、健康调查简表(SF-36),治疗前及治疗15、30 d美国国立卫生研究院卒中量表(NIHSS)评分,并统计2组疗效及不良反应发生情况。结果治疗组总有效率95.99%(47/49),对照组总有效率77.6%(38/49),治疗组疗效优于对照组(P<0.05)。治疗后2组临床症状各项评分均较本组治疗前降低(P<0.05),且治疗后治疗组均低于对照组(P<0.05)。治疗15、30 d 2组NIHSS评分均较本组治疗前降低(P<0.05),治疗30 d均低于本组治疗15 d(P<0.05),且治疗15、30 d治疗组均低于对照组同期(P<0.05)。治疗后2组SF-36各维度评分均较本组治疗前升高(P<0.05),且治疗后治疗组均高于对照组(P<0.05)。治疗组不良反应发生率低于对照组(P<0.05)。结论化痰通腑汤治疗缺血性卒中急性期痰热腑实证患者疗效确切,能有效改善患者临床症状及神经功能缺损情况,不良反应情况较少,生活质量得到明显提升,利于临床推广应用。Objective To explore the clinical effect of Huatan Tongfu Decoction on patients with acute ischemic stroke(AIS)of phlegm-heat and bowel-repletion syndrome and its influence on neurologic impairment and quality-of-life.Methods Totally 98 patients with AIS(phlegm-heat and bowel-repletion syndrome)were randomly divided into equal group,in addition to conventional Western medicine,and treatment group received Huatan Tongfu Decoction.Treated for 10 days,the key observation was clinical symptoms score,36-item Short Form Health Survey(SF-36);National Institutes of Health Stroke Scale(NIHSS)before treatment(day 0),and on the days 15,30.The curative effects and adverse reactions were counted.Results The total effective rates were better in treatment group than in control group[95.99%(47/49)vs 77.6%(38/49),(P<0.05),respectively].After treatment,clinical manifestation scores in groups were decreased(P<0.05),treatment group was decreased in comparison with control group(P<0.05).NIHSS scores in groups on days 15,30 were decreased than those before treatment,which were decreased in treatment group than in concurrent cortrol group(P<0.05);NIHSS scores in groups on day 30 were decreased than on day 15(P<0.05).After treatment,SF-36 dimension scores in groups were increased(P<0.05),which was higher in treatment group than in control group(P<0.05);incidence of adverse reactions was lower in treatment group than in control group(P<0.05).Conclusion For patients with AIS(phlegm-heat and bowel-repletion syndrome),Huatan Tongfu Decoction has a definite curative effect,which is effectively capable of improving clinical symptoms,neurologic impairment,and quality-of-life,with less adverse reactions and being helpful to clinical application.
