摘 要:目的探讨腭部良性肿瘤切除术中应用猪小肠黏膜下层脱细胞修复补片修复硬腭黏膜缺损的临床效果和安全性。方法腭部良性肿瘤患者15例,均行手术治疗,术中应用猪小肠黏膜下层脱细胞修复补片修复硬腭黏膜缺损。术后1、2周及1、3、6个月随访,观察并记录腭部手术区创面及黏膜愈合、补片成活及收缩情况,并发症发生情况。结果 15例患者手术均成功,硬腭黏膜缺损创面均于术后2~4周愈合。术后2周腭部缺损创面可见大量肉芽组织生长,缺损边缘可见新生黏膜组织并向缺损中央方向生长;术后1个月硬腭黏膜缺损区可见大面积黏膜上皮;术后3个月缺损创面已完全被再生黏膜覆盖,呈粉色,质地柔软;术后6个月硬腭缺损创面区黏膜表面颜色、形态、质地与周围黏膜组织接近,平整无瘢痕。术后6个月,黏膜修复优秀者13例,良好者2例;补片成活及收缩情况评价优秀者12例,良好者3例。15例均未发生腭部手术区感染、血肿等严重并发症及排斥反应。结论猪小肠黏膜下层脱细胞修复补片用于修复硬腭黏膜缺损创面可提高黏膜愈合质量,减少并发症发生。Objective To investigate the clinical effect and safety of porcine small intestinal submucosa(SIS) in repairing human hard palatal mucosal defect in palatal benign tumor resection. Methods Fifteen patients with palatal benign tumor received repairment of hard palatal mucosal defect with SIS, and were followed up for 1 and 2 weeks as well as 1, 3 and 6 months after operation to observe the healing of wound and mucosa, the survival and contraction of the SIS patch, and the occurrence of complications. Results All 15 patients got early healing of the wound of hard palatal mucosal defect in 2 to 4 weeks after operation. Two weeks after operation, a large number of granulation tissue was observed in the wound of hard palatal defect, and new mucosal tissue was seen at the edge of the defect, growing towards the center of the defect. One month after operation, the hard palatal mucosal defect was covered with a large of mucosal epithelium. Three months after operation, the wound was completely covered by the soft and pink regenerated mucosa. Six months after operation, the color, shape and texture of the regenerated mucosa of the defect were similar to the adjacent mucosa, with no scar formation. The result of regenerated mucosa evaluation was excellent in 13 patients and good in 2. The survival and contraction of the SIS patch were excellent in 12 patients and good in 3. No serious postoperative complications such as infection, hematoma and rejection reaction occurred. Conclusion The SIS can significantly improve the quality of regenerated mucosa and reduce the occurrence of complications in repairing hard palatal mucosal defect.
