摘 要:目的:探讨急性髓细胞白血病(Acute myeloid leukemia,AML)患儿的miR-126表达水平及临床意义。方法:选取2017年8月~2019年8月在我院诊治的56例AML患儿为AML组,同期48例骨髓穿刺检查的非血液系统肿瘤患儿作为正常组,采用实时荧光定量PCR法检测AML组及对照组骨髓单个核细胞中miR-126的表达水平,比较不同年龄、性别、亚型与miR-126表达的关系,并用ROC曲线评估血清miR-126表达水平对AML的诊断效能。结果:AML组miR-126相对表达水平显著高于正常组(P<0.05);不同性别、年龄AML患儿miR-126相对表达水平对比无明显差异(P>0.05),M3型AML患儿miR-126相对表达水平显著高于非M3型(P<0.05);ROC曲线分析骨髓细胞中miR-126诊断AML的曲线下面积为0.985,95%CI为0.939~0.999,当最佳切点2^(-△△Ct)定在1.104时,miR-126诊断AML的敏感性和特异性分别为98.21%、89.58%。结论:AML患儿中miR-126表达水平显著高于正常组,特别是M3型,且ROC曲线显示诊断效能较高,可将miR-126作为诊断AML的潜在分子标志物。Objective:To explore the expression level and clinical significance of miR-126 in children with acute myeloid leukemia(AML).Methods:A total of 56 AML children who were diagnosed and treated in the hospital from August 2017 to August 2019 were enrolled as AML group,while 48 children with tumors not derived from blood system who underwent bone marrow puncture were enrolled as the normal group.Real-time fluorescence quantitative PCR method was used to detect the miR-126 expression levels in bone marrow mononuclear cells of both groups.The relationship between different age,genders,subtypes and miR-126 expression was compared.The diagnostic efficiency of serum miR-126 for AML was evaluated by ROC curves.Results:The relative expression level of miR-126 in AML group was significantly higher than that in normal group(P<0.05).There was no significant difference in the relative expression level of miR-126 among AML children with different genders and age(P>0.05).The relative expression level of miR-126 in patients with M3 type was significantly higher than that with non-M3 type(P<0.05).ROC curve analysis showed that area under the curve and 95%CI of miR-126 in bone marrow cells for diagnosis of AML were 0.985 and 0.939-0.999,respectively.When the best cut-off value 2^(-△△Ct) was 1.104,the sensitivity and specificity of miR-126 for diagnosis of AML were 98.21%and 89.58%,respectively.Conclusion:The expression level of miR-126 in AML children is significantly higher than that in normal group,especially in those with M3 type.ROC curves show that its diagnostic efficiency is high.miR-126 can be applied as a potential molecular marker for the diagnosis of AML.
