55例脑膜癌脑脊液肿瘤细胞形态特征分析

摘　　要：目的:探讨研究不同恶性肿瘤脑膜转移的脑脊液肿瘤细胞形态特征,对细胞学诊断上的意义。方法:分析研究近3年多以来临床拟诊脑膜癌病55例,临床、影像学资料、首次腰穿脑脊液糖、总蛋白质测定以及肿瘤细胞形态特征。结果:临床资料显示,患者近2~3周来均出现明显的头痛、头晕、记忆力减退且逐渐加重,其中6例伴有恶心、呕吐,2例伴有精神障碍,1例癫痫。影像学示52%脑膜出现强化灶或结节型强化灶。首次腰穿脑脊液白细胞计数、葡萄糖、总蛋白质测定均有不同程度的异常,细胞学检查肿瘤细胞检出阳性率94.5%,未发现原发灶7.3%。结论:脑脊液肿瘤细胞形态特征不仅是细胞学确诊的重要依据,也是判断原发灶的重要参考。Objective:To study the morphological characteristics of cerebrospinal fluid tumor cells with meningeal metastasis of different malignant tumors and its significance in cytological diagnosis.Methods:55 cases of meningeal carcinomatosis were analyzed and studied in recent 3 years,including clinical and imaging data,determination of glucose and total protein in cerebrospinal fluid of the first lumbar puncture and morphological characteristics of tumor cells.Results:Clinical data showed that all patients had obvious headache,dizziness and memory loss in recent 2-3weeks,including 6 cases with nausea and vomiting,2 cases with mental disorder and 1 case with epilepsy.Imaging showed that 52%of meninges had enhancement focus or nodular enhancement focus.The WBC count,glucose and total protein in cerebrospinal fluid of the first lumbar puncture were abnormal in varying degrees.The positive rate of tumor cells detected by cytology was 94.5%,and no primary focus was found in 7.3%.Conclusion:The morphological characteristics of CSF tumor cells are not only an important basis for cytological diagnosis,but also an important reference for judging the primary focus.
