体部伽马刀治疗恶性纤维组织细胞瘤术后肺转移患者的疗效分析

摘　　要：目的探讨体部伽马刀治疗恶性纤维组织细胞瘤(MFH)术后肺转移患者的疗效和不良反应。方法将9例MFH肺转移患者共40个病灶行体部伽马刀治疗,37个病灶以50%剂量线为处方剂量照射,单次剂量3~8 Gy,治疗8~15次;3个病灶以60%剂量线为处方剂量,单次剂量8~10 Gy,治疗5~7次。观察患者的近期疗效、总生存期(OS)、无进展生存期(PFS)及不良反应发生情况。结果随访期间,9例患者生存率为44.44%,PFS为7个月(95%CI:3~12个月),OS为78个月(95%CI:51~134个月),40个病灶治疗总有效率为87.5%。治疗期间,9例患者均未出现3级以上不良反应。结论体部伽马刀治疗MFH术后肺转移患者效果明显,不良反应较轻,是一种有效的治疗手段。Objective To investigate the clinical effect and adverse reaction of body gamma radiotherapy in the treat-ment of postoperative lung metastasis of malignant fibrous histiocytoma(MFH).Method A total of 9 patients with 40 lung metastases were treated by body gamma radiotherapy.The daily radiation dose of 50%isodose line covering meta-static lesions were from 3-8 Gy in 37 lesions for 8-15 times.The daily radiation dose of 60%isodose line covering meta-static lesions were from 8-10 Gy in 3 lesions for 5-7 times.The short-term efficacy,overall survival(OS),progression free survival(PFS)and adverse reactions were observed.Result During the follow-up,the survival rate of 9 patients was 44.44%,the PFS was 7 months(95%CI:3-12 months),the OS was 78 months(95%CI:51-134 months),the total ef-fective rate for 40 lesions was 87.5%.There were no adverse reactions with 3 or more grades.Conclusion The body gamma radiotherapy has good curative effect and less adverse reactions in treating pulmonary metastases of MFH.
