摘　　要：目的:分析复发性流产(Recurrent spontaneous abortion,RSA)患者早孕期外周血VEGF、Mg^(2+)及NK细胞水平与RSA的相关性,为预防治疗RSA提供研究参考依据。方法:选取2019年1月-2020年12月期间本院收治50例初次人工流产妇女为对照组,70例RSA患者作为观察组(RSA组),RSA组根据流产次数再分为RSAⅠ组、RSAⅡ组和RSAⅢ组。对各组患者外周血的VEGF、Mg^(2+)及NK细胞水平检测和数据统计分析。结果:与对照组相比,RSA组外周血的VEGF和Mg^(2+)水平均降低(P<0.05),而NK细胞含量增加(P<0.05)。与RSAⅠ组比较,RSAⅡ组患者外周血中VEGF水平和NK细胞含量差异有统计学意义(P<0.05);RSAⅢ组患者外周血中VEGF、Mg^(2+)水平和NK细胞含量差异均有统计学意义(P<0.05)。与RSAⅡ组比较,RSAⅢ组患者外周血中VEGF水平降低(P<0.05)。VEGF、Mg^(2+)和NK细胞水平检测对RSA患者诊断的AUC分别为0.804、0.773、0.749,三者联合检测AUC为0.874高于其单独预测(P<0.05)。结论:外周血VEGF、Mg^(2+)和NK细胞水平变化与RSA有相关性,且三指标联合监测可以提高诊断价值。因此,积极监测孕妇VEGF、Mg^(2+)和NK细胞水平变化情况及对其进行调控对于预防流产具有重要意义。Objective:To analyze the correlation between the levels of VEGF,Mg^(2+) and NK cells in peripheral blood of patients with recurrent spontaneous abortion(RSA)and RSA in early pregnancy,and to provide reference for the prevention and treatment of RSA.Methods:From January 2019 to December 2020,50 women with first induced abortion in our hospital were selected as the control group,and 70 patients with RSA were selected as the observation group(RSA group).According to the abortion times,RSA group were further classified into RSAⅠgroup,RSAⅡgroup and RSAⅢgroup.The levels of VEGF,Mg^(2+) and NK cells in peripheral blood of each group were detected and statistically analyzed.Results:Compared with the control group,the levels of VEGF and Mg^(2+) in peripheral blood of RSA group were significantly decreased(P<0.05),while the level of NK cells was significantly increased(P<0.05).Compared with the RSAⅠgroup,the changes of VEGF and NK cells were significantly significant in RSAⅡgroup(P<0.05),while the changes of VEGF,Mg^(2+) and NK cells were significantly significant in RSAⅢgroup(P<0.05).Compared with the RSAⅡgroup,the changes of VEGF was significantly significant in RSAⅢgroup(P=0.015).The AUC of VEGF,Mg^(2+) and NK cells were 0.804,0.773 and 0.749,respectively.The AUC of the combined detection of the three was 0.874,which was significantly higher than that of the single one(P<0.05).Conclusion:The levels of VEGF,Mg^(2+) and NK cells in peripheral blood are correlated with RSA,and the combined monitoring of the three indexes can improve the diagnostic value.Therefore,it is of great significance to actively monitor and regulate the changes of VEGF,Mg^(2+) and NK cells in pregnant women for the prevention of abortion.
