摘　　要：目的观察经动脉化疗栓塞术(TACE)联合射频消融术对巨块型肝癌患者的治疗效果及其对谷丙转氨酶(ALT)、谷草转氨酶(AST)及甲胎蛋白(AFP)水平和免疫功能指标的影响。方法选取2018年2月至2019年8月在郑州市第三人民医院治疗的80例巨块型肝癌患者,根据治疗方法的不同分为对照组和观察组,每组40例。对照组给予单纯TACE治疗,观察组在此基础上给予射频消融术。统计两组治疗效果和生存率,比较两组治疗前后肝功能指标、肿瘤标志物指标及免疫功能指标水平情况。结果观察组局部客观缓解率高于对照组(P<0.05)。观察组治疗后ALT、AST、AFP水平低于对照组(P<0.05)。观察组治疗后CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)优于对照组(P<0.05)。结论TACE联合射频消融术对巨块型肝癌患者的治疗效果较好,可明显改善患者的ALT、AST及AFP水平,具有良好的应用价值。【Objective】To observe the therapeutic effect of TACE combined with radiofrequency ablation on patients with massive liver cancer,as well as the effect on AFP,ALT,AST levels,and immune function indicators.【Methods】The clinical data of patients with massive liver cancer who were treated in the Third People's Hospital of Zhengzhou from February 2018 to August 2019 were selected for analysis.According to their different surgical methods,they were divided into control group and observation group.The control group was treated with TACE alone,and the observation group was treated with radiofrequency ablation on this basis.The treatment effect and survival rate of the two groups were counted,and the levels of liver function indexes,tumor marker indexes and immune function indexes were compared between the two groups before and after treatment.【Results】The effective rate of the observation group(97.50%)was higher than that of the control group(82.50%)(P<0.05).After treatment,the ALT,AST,and AFP levels of the observation group were 50.11±2.84 u/L,38.26±3.57 u/L,90.27±8.15μg/L,which were lower than those of the control group(56.34±3.05 u/L,45.11±2.03 u/L,156.48±10.02μg/L)(P<0.05).After treatment,the observation group's CD 3+,CD 4+,CD 4+/CD 8+were(65.36±8.38)%,(39.03±6.14)%,1.78±0.55,which were better than those of the control group,(56.97±5.04)%,(33.36±8.81)%,1.16±0.52,(P<0.05).【Conclusion】TACE combined with radiofrequency ablation has a good therapeutic effect on patients with massive liver cancer.It can significantly improve the levels of ALT,AST and AFP in patients,and has good application value.
