摘  要:目的:探讨声触诊组织量化成像技术(VTIQ)在脑卒中后前臂屈肌痉挛定量评价中的应用价值。方法:采用VTIQ测量102例脑卒中后前臂屈肌痉挛患者双侧前臂屈肌各肌层(旋前圆肌、指浅屈肌、指深屈肌及旋前方肌)放松、紧张时的剪切波速度(SWV),同时采用表面肌电图和改良Ashworth量表(MAS)测量患肢被动牵拉过程中的平均肌电值(AEMG)、均方根值(RMS)及MAS评分,分析SWV值与AEMG值、RMS值及MAS评分之间的相关性。结果:患者双侧各肌层(旋前圆肌、指浅屈肌、指深屈肌及旋前方肌)紧张时的SWV值均高于放松时(P<0.05),患侧放松与紧张时的SWV差值高于健侧(P<0.05)。患侧各肌层SWV值与AEMG值、RMS值及MAS评分均呈正相关(r>0.00,P<0.05)。结论:VTIQ可对脑卒中后前臂屈肌痉挛患者前臂屈肌各肌层进行量化评估,具有较高的临床应用价值。Objective:To investigate the application value of virtual touch tissue imaging quantification(VTIQ)in quantitative evaluation of forearm flexor spasm after stroke.Methods:102 patients with forearm flexor spasm after strokewere selected.VTIQ was used to measure the shear wave velocity(SWV)during relaxation and tension of bilateral forearm flexor muscle layers(pronationa circularis,flexor digitorum superficialis,flexor digitorum profundus and pronationa quadratus).Surface electromyography and modified Ashworth scale(MAS)were used to measure the average electromyography(AEMG),root mean square(RMS)and MAS score during passive traction.The correlation between SWV value and AEMG value,RMS value and MAS score was analyzed.Results:The SWV values of bilateral muscular layers(pronation,flexor digitorum superficialis,flexor digitorum profundus and quadratus pronation)at tension were higher than those at relaxation(P<0.05).The SWV values of affected side at relaxation and tension were significantly higher than those of healthy side(P<0.05).SWV value of each muscle layer on the affected side was positively correlated with AEMG value,RMS value and MAS score(P<0.05).Conclusion:VTIQ can quantitatively evaluate each muscle layer of forearm flexor in patients with forearm flexor spasm after stroke,and has high clinical application value.
