通腑逐瘀汤加减联合颅内血肿微创清除术治疗重症高血压脑出血临床研究

摘　　要：目的:观察通腑逐瘀汤加减联合颅内血肿微创清除术治疗重症高血压脑出血(HICH)的临床疗效。方法:选取42例重症HICH患者,按照就诊时间分为对照组20例和观察组22例。对照组行颅内血肿微创清除术治疗,观察组在对照组基础上给予通腑逐瘀汤加减治疗。比较2组临床疗效及并发症发生率;比较2组治疗前后神经功能缺损程度。结果:观察组总有效率为90.91%,高于对照组的60.00%(P<0.05)。治疗后,2组神经功能缺损评分均较治疗前降低(P<0.05),观察组神经功能缺损评分低于对照组(P<0.05)。观察组恶心、头晕、再出血发生率均低于对照组(P<0.05)。结论:通腑逐瘀汤加减联合颅内血肿微创清除术治疗重症HICH可提高临床疗效,改善患者的神经功能,降低术后并发症发生率。Objective:To observe the clinical effect of modified Tongfu Zhuyu tang combined with minimally invasive surgery of critical hypertensive intracerebral hemorrhage(HICH) due to severe hypertension.Methods:A total of 42 cases of severe critical HICH patients were selected and divided into the control group and the observation group according to their hospitalizing time,with 20 and 22 cases in each group respectively.The control group was treated with minimally invasive surgery of intracranial hematoma,and the observation group was additionally treated with modified Tongfu Zhuyu tang based on the treatment of the control group.The degree of neurological impairment before and after treatment as well as clinical effects and incidence of complications were compared between the two groups.Results:The total effective rate was90.91% in the observation group,higher than that of 60.00% in the control group(P<0.05).After treatment,scores of neurological impairment in the two groups were decreased when compared with those before treatment(P<0.05),and the score in the observation group was lower than that in the control group(P<0.05).The incidence of nausea,dizziness and rebleeding in the observation group was lower than that in the control group(P<0.05).Conclusion:Modified Tongfu Zhuyu tang combined with minimally invasive surgery of intracranial hematoma for critical HICH can improve the clinical effect and neurological function,and reduce the incidence of postoperative complications.
