胃癌患者组织和血清外泌体中lncRNA NEAT1表达及临床意义

摘　　要：目的探讨胃癌患者组织和血清外泌体中长链非编码RNA核富集转录体1(lncRNA NEAT1)的表达及其诊断价值。方法收集2019年1月至12月于我院行手术的胃癌患者90例作为研究组,另纳入胃部良性病变患者30例和健康志愿者30例作为对照组,取研究组患者肿瘤组织、癌旁组织以及提取所有受试者的静脉血血清外泌体,采用实时荧光定量PCR(qPCR)法检测血清外泌体、肿瘤组织及癌旁组织中的lncRNA NEAT1表达水平,采用受试者工作特征曲线(ROC)分析血清外泌体lncRNA NEAT1对胃癌的诊断价值;分析lncRNA NEAT1与临床病理特征和常规血清肿瘤标志物(CEA、CA199、CA125)的关系。结果lncRNA NEAT1在胃癌组织中的相对表达量为2.35±0.86,高于癌旁组织的1.00±0.33(P<0.05);胃癌患者血清外泌体中lncRNA NEAT1相对表达量高于正常人群和良性病变组(1.52±0.36 vs.1.00±0.17 vs.1.09±0.20),差异具有统计学意义(P<0.05);胃癌患者血清外泌体和胃癌组织中lncRNA NEAT1表达呈正相关性(r=0.687,P<0.001);血清外泌体lncRNA NEAT1诊断胃癌的ROC曲线下面积(AUC)、灵敏度、特异度分别为0.876(95%CI:0.823~0.929)、83.67%、67.78%;血清外泌体和组织中lncRNA NEAT1表达量与与肿瘤直径、淋巴结转移、临床分期、HP感染有关(P<0.05);胃癌组患者血清外泌体lncRNA NEAT1相对表达量与血清CEA、CA199、CA125水平呈正相关(r=0.587、r=0.457、r=0.394,P<0.001)。结论胃癌患者组织和血清中外泌体中lncRNA NEAT1表达量显著上调,并且血清外泌体lncRNA NEAT1未来作为胃癌的辅助诊断标志物具有很大的潜力。Objective To investigate the expression and diagnostic value of long-chain noncoding RNA nuclear enriched transcript 1(lncRNA NEAT1)in tissue and serum exosomes of patients with gastric cancer.Methods Ninety patients with gastric cancer who underwent surgery in our hospital from January to December 2019 were collected as the study group,and 30 subjects with benign gastric lesions and 30 healthy volunteers were included as the control group.The tumor tissues,adjacent tissues and venous blood serum exosomes of all subjects in the study group were taken,and the serum exosomes were detected by real-time fluorescence quantitative PCR(qPCR).The expression level of lncRNA NEAT1 in tumor tissues and adjacent tissues was analyzed by subject operating characteristic curve(ROC).The relationship between lncRNA NEAT1 and clinicopathological features and routine serum tumor markers(CEA,CA199,CA125)was analyzed.Results The relative expression of lncRNA NEAT1 in gastric cancer was 2.35±0.86,which was higher than 1.00±0.33 in adjacent tissues(P<0.05).The relative expression of lncRNA NEAT1 in serum exosomes of patients with gastric cancer was significantly higher than that in normal population and benign lesion group(1.52±0.36 vs.1.00±0.17 vs.1.09±0.20,P<0.05).There was a positive correlation between the expression of lncRNA NEAT1 in serum exosomes and gastric cancer tissues(r=0.687,P<0.001).The area under ROC curve(AUC),sensitivity and specificity of serum secreted lncRNA NEAT1 in the diagnosis of gastric cancer were 0.876(95%CI:0.823-0.929),83.67%and 67.78%,respectively.The expression of lncRNA NEAT1 in serum exosomes and tissues was related to tumor diameter,lymph node metastasis,clinical stage and HP infection(P<0.05).The relative expression of serum secreted lncRNA NEAT1 in gastric cancer group was positively correlated with the levels of serum CEA,CA199 and CA125(r=0.587,r=0.457,r=0.394,P<0.001).Conclusion The expression of lncRNA NEAT1 in exosomes in tissues and serum of patients with gastric cancer is significantly u
