柳州市孤独症谱系障碍早期筛查的三级网络建设及其效果

摘　　要：目的建设柳州市孤独症谱系障碍(ASD)早期筛查三级网络系统,分析系统建立后的效果。方法建立社区卫生服务中心(卫生院)为一级筛查机构,县(区)妇幼保健院为二级筛查机构,市妇幼保健院为三级专科诊断和干预机构,比较三级筛查网络系统建立后各监测指标的变化。结果三级筛查网络建立后,ASD的一级筛查率由2019年的71.5%上升到2020年的85.0%,一级筛查单位向二级筛查单位转诊到位率由2019年的57.8%上升到2020年的85.4%,均呈上升趋势;二级筛查单位向三级诊断机构转诊到位率由2019年的82.4%上升到2020年的89.9%;早期确诊的ASD患儿人数由2019年的26人上升到2020年的58人,平均确诊年龄是(1.4±0.6)岁。结论 ASD早期筛查三级网络建立后,柳州市ASD儿童得到早期筛查、诊断和干预,预后改善,值得推广应用。Objective To construct a three-level network system for early screening of autism spectrum disorder(ASD) in Liuzhou, and to analyze the effect of the system. Methods Community health service centers(health centers) were established as the first-level screening institutions, county(district) maternal and child health hospitals as the second-level screening institutions, and municipal maternal and child health hospitals as the third-level specialized diagnosis and intervention institutions. The changes of the monitoring indicators after the establishment of the three-level screening network system were compared. Results After the establishment of the three-level screening network, the primary screening rate of ASD increased from 71.5% in 2019 to 85.0% in 2020, and the referral rate from primary screening units to secondary screening units increased from 57.8% in 2019 to 85.4% in 2020, both showing an upward trend. The referral rate from secondary screening units to tertiary diagnostic institutions increased from 82.4% in 2019 to 89.9% in 2020. The number of children diagnosed early with ASD increased from 26 in 2019 to 58 in 2020, and the average age of diagnosis was(1.4±0.6) years. Conclusion After the establishment of the three-level network for early screening of ASD, children with ASD in Liuzhou received early screening, diagnosis and intervention, and the prognosis is improved, which is worthy of promotion and application.
