翼状胬肉切除联合自体结膜瓣移植术后疼痛程度的影响因素

摘　　要：目的探讨翼状胬肉切除联合自体结膜瓣移植术后疼痛程度的影响因素,以期为临床治疗提供参考。方法回顾性分析2019年1月至2021年1月在厦门大学附属中山医院眼科接受翼状胬肉切除联合自体结膜瓣移植术且符合条件的157例患者的临床资料,采用单因素和多因素logistic回归分析翼状胬肉切除联合自体结膜瓣移植术后明显疼痛的影响因素。结果 157例患者经翼状胬肉切除联合自体结膜瓣移植术后观察24小时,42例出现明显疼痛,发生率为26.8%。多因素logistic回归分析发现,翼状胬肉头部侵入角膜过大、创面累及角膜基质层、手术时间过长是翼状胬肉切除联合自体结膜瓣移植术后出现明显疼痛的独立危险因素(P<0.05)。结论翼状胬肉侵入角膜过大、角膜创面层次过深、手术时间过长均可导致患者术后发生明显眼痛,临床可通过相应改进措施,减轻患者不适。Objective To explore the influencing factors of pain after pterygium excision combined with autologous conjunctival transplantation, so as to provide reference for clinical treatment. Methods The clinical data of 157 patients who received pterygium excision combined with autologous conjunctival transplantation in the Department of Ophthalmology of Zhongshan Hospital Affiliated to Xiamen University from January 2019 to January 2021 were retrospectively analyzed. Univariate analysis and multivariate logistic regression analysis were performed for the influencing factors of obvious pain after pterygium excision combined with autologous conjunctival transplantation. Results The 157 patients included in the study were observed 24 hours after the pterygium excision combined with autologous conjunctival transplantation. 42 of them had obvious pain, and the incidence was 26.8%. Multivariate logistic regression analysis showed that, excessive invasion of pterygium head into cornea, wound involving corneal stroma and long operation time were the independent risk factors for the obvious pain after pterygium excision combined with autologous conjunctival transplantation(P<0.05). Conclusion Excessive invasion of pterygium head into cornea, deep corneal wound layer and long operation time can lead to obvious postoperative eye pain. Corresponding improvement measures can be taken to reduce the discomfort of patients in clinical practice.
