乳腺锥光束CT平扫与数字乳腺X线摄影对乳腺病灶检出及定性比较

摘　　要：目的探讨锥光束乳腺CT(CBBCT)平扫与数字乳腺X线摄影(DM)对乳腺病灶的检出情况及定性情况,研究CBBCT平扫是否较DM更适于临床诊断需要。方法回顾性分析2020年10月至2021年1月同时接受CBBCT平扫和DM检查的患者,患者均为女性且行病理检查。结果 CBBCT平扫共检出83个病灶,DM共检出67个病灶,且CBBCT平扫较DM有更高特异度(92.1%vs.75.0%)、灵敏度(93.3%vs.86.6%)、粗一致率(92.8%vs.82.1%)、曲线下面积(AUC)值(0.961 vs.0.876)及约登指数(0.85 vs.0.64)。CBBCT平扫及DM对于良性病变的检出率及恶性病变的检出率均有显著差异;部分经DM检查后经CBBCT平扫检查乳腺影像报告和数据系统(BI-RADS)分类发生明显变化。结论 CBBCT平扫较DM对乳腺病灶的诊断效能更高,且CBBCT平扫在评估DM不确定的BI-RADS分类(0、3、4)方面的具有更高优势。推荐在临床发现乳腺病变时,优先选择CBBCT平扫。Objective To explore the detection and qualitative situation of breast lesions with NC-CBBCT and DM,and to investigate whether NC-CBBCT is more suitable for clinical diagnosis than DM. Methods Retrospective analysis was performed on the patients who received both NC-CBBCT and DM examination from October 2020 to January 2021.All the patients were female and underwent pathological examination. Results A total of 85 lesions were detected by NC-CBBCT,and 66 lesions were detected by DM.Compared with DM,NC-CBBCT had higher specificity(92.1%/75.0%),sensitivity(93.3%/86.6%),rough consistency(92.8%/82.1%),AUC(0.961/0.876)and Youden index(0.85/0.64).There were significant differences in the detection rate of benign lesions and malignant lesions between CBBCT and DM.After DM examination, the classification of BI-RADS was significantly changed by NC-CBBCT. Conclusion NC-CBBCT is more effective than DM in the diagnosis of breast lesions, and NC-CBBCT has a higher advantage in the evaluation of DM uncertain BI-RADS classification(0,3,4).We recommend NC-CBBCT rather than DM in clinical diagnosis.
