内镜下套扎联合组织胶“三明治”注射治疗EVB患者的效果及安全性

摘　　要：目的探讨内镜下套扎联合组织胶“三明治”注射治疗食管胃底静脉曲张破裂出血(EVB)的效果及安全性。方法选取该院收治的EVB患者124例作为研究对象,采用随机数字表分为研究组和对照组各62例,研究组采用内镜下套扎联合组织胶“三明治”注射治疗,对照组采用内镜下套扎联合聚桂醇注射治疗;对比两组患者急诊止血成功率、治疗效果、静脉套扎次数、注射点数、手术前后患者血常规及凝血功能指标的差异。结果研究组患者的急诊止血成功率95.16%高于对照组的82.26%(P<0.05),研究组的静脉套扎次数、注射点数与对照组比较,差异无统计学意义(P>0.05);术后6个月进行临床疗效评估,研究组治疗显效率69.35%、有效率29.03%、无效率1.61%,对照组治疗显效率50.00%、有效率41.94%、无效率8.06%,两组差异有统计学意义(P<0.05);术前,研究组和对照组的Hb、WBC、PLT、PT、TT、APTT水平差异无统计学意义(P>0.05);术后3个月,研究组和对照组的Hb、WBC、PLT、PT、TT、APTT水平差异无统计学意义(P>0.05);两组患者术后3个月的Hb、PLT测定值较本组术前显著提高(P<0.05),PT、APTT测定值较本组术前显著降低(P<0.05);研究组患者的手术并发症率9.68%低于对照组的24.19%(P<0.05)。结论内镜下套扎联合组织胶“三明治”注射治疗EVB患者的效果优于内镜下套扎联合聚桂醇注射的效果。Objective To explore the effect and safety of endoscopic band ligation combined with tissue glue"sandwich"injection in the treatment of esophageal and gastric varices bleeding(EVB).Methods A total of 124 cases of EVB patients admitted to our hospital were selected as the research objects.They were divided into the study group and the control group with 62 cases each by using a random number table.The study group was treated with endoscopic band ligation combined with tissue glue"sandwich"injection,and the control group was treated with endoscopy Lower band ligation combined with cinnamyl alcohol injection treatment;The difference in the success rate of emergency hemostasis,treatment effect,venous ligation times,injection points,blood routine and coagulation function indexes before and after surgery were compared between the two groups.Results The success rate of emergency hemostasis in the study group was 95.16%higher than that in the control group(P<0.05).There was no significant difference in the number of venous ligation and injection points between the study group and the control group(P>0.05);The clinical efficacy was evaluated within months.The effective rate of treatment in the study group was 69.35%,the effective rate was 29.03%,and the inefficiency was 1.61%.In the control group,the effective rate was 50.00%,the effective rate was 41.94%,and the inefficiency was 8.06%.The difference between the two groups was statistically significant.(P<0.05);Before operation,there was no significant difference in Hb,WBC,PLT,PT,TT,APTT between the study group and the control group(P>0.05);3 months after the operation,the study group and the control group The Hb,WBC,PLT,PT,TT,APTT measured values were not statistically different(P>0.05);the Hb and PLT measured values of the two groups of patients 3 months after surgery were significantly higher than those of this group before surgery(P<0.05)),the measured values of PT and APTT were significantly lower than those of this group before surgery(P<0.05);the surgical complicat
