摘 要:目的探讨三维可视化手术规划系统在微波消融治疗肝脏血管瘤患者的效果。方法选取2018年10月至2020年4月本院收治的采用微波消融治疗的肝脏血管瘤患者66例作为研究对象,以双盲法分为对照组和实验组,各33例。对照组采用二维可视化手术规划系统治疗,实验组采用三维可视化手术规划系统治疗,比较两组治消融指标、肝功能指标及并发症发生情况。结果实验组消融能量低于对照组,消融时间短于对照组,完全消融率高于对照组,差异均有统计学意义(P<0.05),但组间消融点数比较差异无统计学意义。消融前,两组ALT及AST指标比较差异无统计学意义;消融3 d后,实验组ALT及AST指标均明显低于对照组,差异有统计学意义(P<0.05)。实验组并发症发生率低于对照组,但差异无统计学意义。结论肝脏血管瘤患者采用微波消融治疗时,应用三维可视化手术规划系统可进一步提高完全消融率,缩短消融时间,降低消融能量,且并发症发生率较低,可显著降低肝功能损伤,治疗效果显著。Objective To investigate the effect of three-dimensional visualization surgical planning system in the treatment of patients with liver hemangioma by microwave ablation.Methods 66 patients with hepatic hemangioma treated by microwave ablation in our hospital from October 2018 to April 2020 were selected as the research subjects,and they were divided into the control group and the experimental group by double-blind method,with 33 cases in each group.The control group was treated with two-dimensional visualization surgical planning system,and the experimental group was treated with three-dimensional visualization surgical planning system.The ablation indexes,liver function indexes and complications were compared between the two groups.Results The ablation energy of the experimental group was lower than that of the control group,the ablation time was shorter than that of the control group,and the complete ablation rate was higher than that of the control group(P<0.05),but there was no significant difference in ablation points between groups.Before ablation,there was no significant difference in ALT and AST between the two groups;three days after ablation,the indexes of ALT and AST in the experimental group were significantly lower than those in the control group(P<0.05).The incidence of complications in the experimental group was lower than that in the control group,but the difference was not statistically significant.Conclusion For patients with hepatic hemangioma,in their microwave ablation treatment,the application of a three-dimensional visualization surgical planning system can further improve the complete ablation rate,shorten ablation time,reduce ablation energy,and have a low complication rate,which can significantly reduce the liver function damage,and the therapeutic effect was remarkable.
