多层螺旋CT与胸部X线诊断早期肺癌的效果观察

摘　　要：目的探讨多层螺旋CT与胸部X线诊断早期肺癌的应用效果。方法选取2020年3月至2021年2月间延安大学附属医院收治的72例疑似早期肺癌患者,均行胸部X线和多层螺旋CT检查,病理学诊断结果为金标准,比较两种检查方式所用时间、费用、征象检出率和诊断结果。结果多层螺旋CT检查的检查时间比胸部X线长,差异有统计学意义(P <0.05),检查费用更高。多层螺旋CT检查的各征象检出率均较胸部X线更高,差异有统计学意义(均P <0.05)。72例疑似患者中,经病理学诊断证实60例(83.3%)为早期肺癌,其中中央型34例,周围型26例,其余12例为其他肺部疾病。胸部X线检查检出阳性49例,阴性7例,误诊漏诊16例。多层螺旋CT检查检出阳性57例,阴性10例,误诊漏诊5例。多层螺旋CT检查的诊断敏感度、准确性和阴性预测值均较胸部X线检查高,差异有统计学意义(均P <0.05)。结论同胸部X线比较,多层螺旋CT检查早期肺癌诊断效能更高,但检查时间长、价格高,可结合实际情况选择检查方式。Objective To study the efficacy of multi-slice spiral CT and chest X-ray for the diagnosis of early stage lung cancer. Methods Seventy-two patients with suspected early stage lung cancer were selected at Yan’an University Affiliated Hospital from March 2020 to February 2021. All subjects underwent chest X-ray examination and multi-slice spiral CT examination. Pathology diagnosis results were considered as the gold standard. Duration,expense,detection rate and diagnosis results were compared between the two inspection methods. Results The duration of multi-slice spiral CT examination was longer than that of chest X-ray examination,and the expense was greater than that of chest X-ray examination(all P < 0. 05).However,the detection rate of multi-slice spiral CT was higher than that of chest X-ray examination(P <0. 05). Sixty participants(83. 3%) were confirmed with early stage lung cancer by pathological diagnosis results including 34 patients with central type tumors and 26 patients with peripheral type tumors and the remaining 12 patients had other lung diseases. Forty participants tested positive and 7 participants tested negative in the chest X-ray,and misdiagnosis and missed diagnosis occurred in 16 participants. Fifty-seven participants had positive chest X-ray findings and 10 participants had negative chest X-ray findings,and misdiagnosis and missed diagnosis occurred in 5 participants. The diagnostic sensitivity,accuracy,and negative predictive value of multi-slice spiral CT were higher than those of chest X-ray(all P < 0. 05). Conclusion Compared with chest X-ray examination,multi-slice spiral CT examination shows high diagnostic efficiency for early stage lung cancer,but the duration is relatively long and the expense is relatively high. Therefore,the detection method should be selected according to the actual situation.
