摘　　要：目的探讨集束化护理干预对前列腺癌患者术后盆底功能锻炼依从性及尿失禁的影响。方法选取2020年1月至2020年12月间上海中医药大学附属曙光医院收治的50例前列腺癌根治术患者作为研究对象。其中,围手术期采用常规护理的20例患者纳入对照组。围手术期采用集束化护理的30例患者纳入观察组。比较两组术后盆底功能锻炼的依从性及尿失禁情况。结果观察组拔管后1个月、3个月盆底功能锻炼的依从性分别为76.7%和70.0%,均高于对照组的45.0%和35.0%,差异均有统计学意义(均P <0.05)。观察组拔管后1个月、3个月的尿失禁发生率分别为10.0%和6.7%,低于对照组的40.0%和35.0%,差异均有统计学意义(均P <0.05)。观察组尿失禁持续时间为(3.83±0.95)天,短于对照组的(5.38±1.97)天,观察组拔管后3个月尿失禁问卷评分低于对照组,差异均有统计学意义(均P <0.05)。结论集束化护理干预能够提高前列腺癌患者术后盆底功能锻炼的依从性,预防术后尿失禁的发生。Objective To explore the effects of cluster nursing intervention on patient compliance with pelvic floor functional exercise and urinary incontinence in patients undergoing prostate surgery.Methods Fifty patients undergoing radical prostatectomy were selected Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 2020 to December 2020. Twenty patients receiving perioperative cluster nursing were included in an observation group and 30 patients receiving conventional nursing in the perioperative period were included in a control group. Patient compliance with pelvic floor functional exercise and urinary incontinence were compared between the two groups. Results Patient compliance with pelvic floor function exercise was 76. 7% and 70. 0%,respectively at 1 month and 3 month after extubaiton in observation group,which was higher than 45. 0% and 35. 0%,respectively of the control group(all P < 0. 05). The incidence of incontinence was 10. 0% and 6. 7%,respectively at 1 month and 3 month after extubaiton in observation group,which was lower than 40. 0% and 35. 0%,respectively of the control group(all P < 0. 05). The duration of incontinence was 3. 83 ± 0. 95 d in observation group which was shorter than 5. 38 ± 1. 97 d of the control group(P < 0. 05). Questionnaire score on urinary incontinence was was lower in the observation group than in the control group,the difference is statistically significant(P < 0. 05). Conclusion Cluster nursing intervention can improve patient compliance with pelvic floor functional exercise and prevent the occurrence of postoperative urinary incontinence in patients with prostate cancer.
