摘 要:目的:评估晚期胃癌(AGC)患者化疗后肿瘤原发灶大小和肿瘤代谢改变与无进展生存期(PFS)和总体生存期(OS)的相关性。方法:回顾性比较120例辅助化疗AGC患者化疗前后18F-FDG PET/CT检查的肿瘤直径、最大标准摄取值(SUVmax)和病灶糖酵解总量(TLG),分析肿瘤代谢变化与肿瘤大小之间的关系,以及二者对AGC患者生存率的影响。结果:基线状态下AGC患者肿瘤大小与SUVmax(r=0.43,P<0.001)、TLG(r=0.84,P<0.001)相关,化疗后AGC患者的肿瘤大小的改变与SUVmax(r=0.48,P<0.001)、TLG(r=0.46,P<0.001)的改变也呈正相关。化疗后肿瘤大小减少合并SUVmax降低或肿瘤大小减小合并TLG降低的患者的PFS和OS明显优于仅有一项参数指标减少或均不减少的患者。结论:化疗介导的肿瘤代谢改变与AGC患者的肿瘤大小改变相关,且肿瘤大小和肿瘤代谢状态改变有助于判断患者预后。Objective:To evaluation of correlation between the changes in tumor size and tumor metabolism status with progression-free survival(PFS)and overall survival(OS)in patients with advanced gastric cancer(AGC)after chemotherapy.Methods:This study retrospectively analyzed the diameter of the maximum standardized uptake value(SUVmax)and total lesion glycolysis(TLG)changes by 18F-FDG PET/CT scans before and after chemotherapy in 120 cases of AGC patients.The association between the changes in tumor size and tumor metabolism was analyzed,and perform the influences of the changes of tumor size and tumor metabolism for the survival of AGC patients.Results:The tumor size of AGC patients showed a high correlation with SUVmax(r=0.43,P<0.001)and TLG(r=0.84,P<0.001)at baseline level.The changes of tumor size after chemotherapy in patients with AGC were positively correlated with the changes of SUVmax(r=0.48,P<0.001)and TLG(r=0.46,P<0.001).After chemotherapy,patients with both tumor size and SUVmax decreased or both tumor size and TLG decreased showed better PFS and OS than those with only either one parameter reduction or no reduction.Conclusion:Tumor metabolic changes mediated by chemotherapy are correlated with changes in tumor size in patients with AGC.Considering both changes in tumor size and tumor metabolism status may help to predict mo presice long-term survival in AGC patients.
