低温等离子射频消融术治疗早期声门型喉癌患者的临床效果研究

摘　　要：目的探究低温等离子射频消融术(RFA)治疗早期声门型喉癌患者的临床效果。方法选择82例早期声门型喉癌患者,采用随机数字表法分为两组各41例。对照组采用传统手术治疗,研究组采用RFA治疗。对比两组手术相关指标、手术前后嗓音声学功能指标及并发症。结果研究组手术时间为(15.59±2.53)min、住院时间为(5.03±1.12)d、黏膜恢复时间为(4.43±0.97)d,均短于对照组的(35.61±5.84)min、(7.94±1.58)d、(6.95±0.86)d,有统计学意义(P<0.05),两组术中出血量比较,无统计学意义(P>0.05);研究组术后基频微扰为(0.26±0.05)%、振幅微扰为(1.82±0.41)%、标准化噪声能量为(-13.30±2.03)dB,均低于对照组的(0.36±0.10)%、(2.38±0.55)%、(-11.98±2.04)%,有统计学意义(P<0.05),两组基频比较,无统计学意义(P>0.05);研究组并发症发生率为4.88%,低于对照组的19.51%,有统计学意义(P<0.05)。结论 RFA在早期声门型喉癌治疗中疗效确切,对患者造成损伤小、术后嗓音恢复好,利于促进预后恢复,值得推广应用。Objective To investigate the clinical effect of low temperature plasma radiofrequency ablation(RFA) in the treatment of early glottic laryngeal carcinoma.Methods 82 patients with early glottic laryngeal carcinoma were selected and divided into 2 groups, 41 cases each.The control group was treated with conventional surgery, RFA therapy was used in the study group.The related indexes, voice acoustic function indexes and complications before and after operation were compared between the 2 groups.Results The operative time of the study group was(15.59±2.53) min、 hospitalization time was(5.03±1.12) d、 mucosal recovery time was(4.43±0.97) and were shorter than those in the control group(35.61±5.84) min、(7.94±1.58) d、(6.95±0.86) d,(P<0.05), intraoperative blood loss between the 2 groups had no statistical significance(P>0.05);The fundamental frequency perturbation(0.26±0.05),amplitude(1.82±0.41),normalized noise energy(-13.30±2.03) were dB in the study group, which were lower than the control group,(0.36±0.10)%,(2.38±0.55)%,(-11.98±2.04)%,(P<0.05),Two sets of fundamental frequency had no statistical significance(P>0.05);The incidence of complications in the study group was 4.88%,Less than 19.51% in the control group(P<0.05).Conclusion RFA in the treatment of early glottic laryngeal carcinoma has accurate curative effect and small injury, the voice recovery is good after operation, it is beneficial to the prognosis recovery, and it is worth popularizing and applying.
