食管癌患者根治手术前后外周血中GATA3、Foxp3 mRNA的表达及其意义

摘　　要：目的研究食管癌患者根治手术前后外周血中GATA结合蛋白3(GATA3)、叉头蛋白P3(Foxp3)mRNA的表达及其意义。方法选取196例食管癌患者为病例组,另选同期的60例体检健康者作为正常组,采用定量PCR检测手术前后患者外周血单核细胞中GATA3、Foxp3 mRNA表达,用流式细胞仪检测外周血样本中CD_(3)^(+)、CD _(4)^(+)T细胞的表达水平,分析GATA3、Foxp3 mRNA表达与食管癌临床病理特征及患者预后的关系。结果病例组手术前的GATA3、Foxp3 mRNA表达水平高于手术后1周和正常组,CD_(3)^(+)、CD _(4)^(+)T细胞水平低于手术后1周和正常组(P<0.05);病例组手术后1周与正常组的GATA3、Foxp3 mRNA、CD_(3)^(+)、CD _(4)^(+)T细胞水平比较,差异无统计学意义(P>0.05)。不同TNM分期、病理分级、肿瘤大小和淋巴结转移的食管癌患者术前GATA3 mRNA表达水平比较,差异有统计学意义(P<0.05);不同TNM分期、淋巴结转移的食管癌患者术前Foxp3 mRNA表达水平比较,差异有统计学意义(P<0.05)。不同TNM分期、病理分级、肿瘤大小、淋巴结转移、GATA3、Foxp3 mRNA表达的食管癌患者1年生存率比较,差异有统计学意义(P<0.05);TNM分期晚(RR=1.610)、Foxp3高表达(RR=1.473)是影响食管癌患者预后的危险因素,差异有统计学意义(P<0.05)。结论食管癌患者术前的GATA3、Foxp3 mRNA呈现高表达,行根治术后水平有所下降。临床应重视Foxp3的高表达,其对患者的预后有重要意义。Objective To study the expression and significance of GATA binding protein 3(GATA3)and forkhead box P3(Foxp3)mRNA in peripheral blood before and after radical surgery of esophageal cancer.Methods 196 patients with esophageal cancer were enrolled as case group,while another 60 healthy controls were enrolled as normal group.The expression of GATA3 and Foxp3 mRNA in peripheral blood monouclear cell(PBMC)before and after surgery was detected by quantitative PCR.The expression levels of CD _(3)^(+)and CD _(4)^(+)T cells in peripheral blood samples were detected by flow cytometry.The relationship between GATA3,Foxp3 mRNA and clinicopathological characteristics,prognosis of esophageal cancer patients was analyzed.Results The expression levels of GATA3 and Foxp3 mRNA before surgery in case group were higher than those at 1 week after surgery and in normal group,while levels of CD _(3)^(+)and CD _(4)^(+)T cells were lower than those at 1 week after surgery and in normal group(P<0.05).There were no significant differences in the levels of GATA3,Foxp3 mRNA,CD _(3)^(+) and CD _(4)^(+)T cells at 1 week after surgery between case group and normal group(P>0.05).The differences in expression level of GATA3 mRNA were statistically significant among esophageal cancer patients with different TNM staging,pathological grading,tumor size and lymph node metastasis(P<0.05).The differences in expression level of Foxp3 mRNA were statistically significant among esophageal cancer patients with different TNM staging and lymph node metastasis(P<0.05).The differences in 1-year survival rate were statistically significant among esophageal cancer patients with different TNM staging,pathological grading,tumor size,lymph node metastasis,GATA3 and Foxp3 mRNA expression(P<0.05).Late TNM staging(RR=1.610)and high-expression(RR=1.473)of Foxp3 were risk factor affecting prognosis of esophageal cancer patients(P<0.05).Conclusion GATA3 and Foxp3 mRNA are highly expressed in esophageal cancer patients,which decrease after radical surgery.Clinically,high-e
