摘　　要：目的探究食管癌切除术后重度肾损伤的影响因素。方法选择行食管癌根治术的124例患者作为研究对象,根据患者术后是否发生重度肾损伤将患者分为重度肾损伤组(18例)及无肾损伤组(106例)。分析影响肿瘤切除术后重度肾损伤的相关因素。结果重度肾损伤组术后肺部感染、术后肾外器官衰竭、合并T2DM、合并EH、发生率高于无肾损伤组,术中淋巴结清扫数大于无肾损伤组,差异有统计学意义(P<0.05);术后肺部感染、术后肾外器官衰竭、合并T2DM、合并EH是影响肿瘤切除术后重度肾损伤的独立危险因素(OR值≥1,且P<0.05)。结论术后肺部感染、肾外器官衰竭、合并T2DM、合并EH是影响食管癌术后重度肾损伤的独立危险因素,在围术期中需予以重视,以降低重度肾损伤发生率,改善预后。Objective To explore the influencing factors of severe renal injury after esophageal cancer resection.Methods 124 patients with esophageal cancer who underwent radical operation were selected as the research objects.According to whether the patients had severe renal injury after operation, the patients were divided into severe renal injury group(18 cases) and no renal injury group(106 cases).Related factors of severe renal injury after tumor resection were analyzed.Results The incidence of postoperative pulmonary infection, postoperative extrarenal organ failure, combined with T2 DM,combined with EH in severe renal injury group was higher than that in non renal injury group, and the number of lymph node dissection in severe renal injury group was higher than that in non renal injury group, the difference was statistically significant(P<0.05);Postoperative pulmonary infection, postoperative extrarenal organ failure, T2 DM and eh were independent risk factors for severe renal injury after tumor resection(or≥1,P<0.05).Conclusion Postoperative pulmonary infection, extrarenal organ failure, combined with T2 DM and eh are independent risk factors for severe renal injury after esophageal cancer surgery.We should pay attention to them in the perioperative period to reduce the incidence of severe renal injury and improve the prognosis.
