摘  要:目的比较胰胆管逆行注射牛黄胆酸钠与腹腔注射L-精氨酸两种重症急性胰腺炎(SAP)造模方式对胰外器官损害的差异。方法18只雄性远交群(SD)大鼠经随机数表法分为3组,每组6只:空白对照组(A组),L-精氨酸诱导SAP模型组(B组),牛磺胆酸钠诱导SAP模型组(C组)。造模后,分别于12、24 h割尾采血,检测谷草转氨酶(AST)、谷丙转氨酶(ALT)、尿素氮(BUN)、肌酐(Cr)、肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)、淀粉酶、内毒素、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)含量,并于24 h取胰腺、肝、回结肠、肾、肺、心、脑组织,进行病理学评分。组间比较采用单因素方差分析。结果(1)12 h,B组和C组的AST(F=203.345,P<0.05)、ALT(F=303.403,P<0.05)、BUN(F=72.921,P<0.05)、Cr(F=205.601,P<0.05)、cTnI(F=35.991,P<0.05)、CK-MB(F=42.815,P<0.05)、淀粉酶(F=1300.111,P<0.05)、内毒素(F=34.158,P<0.05)、IL-6(F=541.858,P<0.05)、TNF-α水平(F=191.687,P<0.05)显著高于A组,差异均有统计学意义;24 h,B组和C组的AST、ALT、BUN、Cr、cTnI、CK-MB、淀粉酶、内毒素、IL-6、TNF-α水平显著高于A组。与12 h比较,B组的AST(189.33±23.47比65.83±6.08,F=155.732,P<0.05)、ALT水平(156.33±10.67比55.33±4.72,F=449.603,P<0.05)高于C组,但BUN(8.80±0.56比11.02±1.33,F=14.131,P<0.05)、Cr(35.30±2.65比58.77±3.65,F=162.136,P<0.05)、cTnI(0.056±0.107比0.070±0.089,F=5.972,P<0.05)、CK-MB(5.23±0.65比6.22±0.70,F=6.426,P<0.05)、淀粉酶(1445.89±70.37比2063.25±72.58,F=223.767,P<0.05)、内毒素(34.00±4.70比47.09±5.90,F=18.030,P<0.05)、IL-6(102.18±7.01比147.45±7.70,F=113.465,P<0.05)、TNF-α水平(152.35±14.43比211.95±19.27,F=36.790,P<0.05)低于C组,差异均有统计学意义。与24 h比较,B组的AST、ALT高于C组,BUN、Cr、cTnI、CK-MB、淀粉酶、内毒素、IL-6、TNF-α水平低于C组。(2)B组、C组中胰腺(F=52.612,P<0.05)、肝脏(F=9.767,P<0.05)、回肠(F=18.879,P<0.05)、结肠(F=31.398,P<0.05)、肾脏(F=54.266,P<0.05)�Objective To compare the damage to extrapancreatic organs by retrograde injection of sodium taurocholate into pancreatic-bile duct and intraperitoneal injection of L-arginine in severe acute pancreatitis(SAP)models.Methods Totally,18 male Sprague Dawley(SD)rats were divided into three groups with 6 rats in each group by random number table method:group A(blank control group),group B(L-arginine-induced SAP model group),and group C(sodium taurocholate-induced SAP model group).After modeling,blood samples were collected by cutting the tail of rats at 12 and 24 h,and the contents of aspartate transaminase(AST),alanine transaminase(ALT),blood urea nitrogen(BUN),creatinine(Cr),cardiac troponin I(cTnI),creatine kinase-MB(CK-MB),amylase,endotoxin,interleukin(IL)-6 and tumor necrosis factor-α(TNF-α)were determined.At 24 h,the tissues of pancreas,liver,ileocolon,kidney,lung,heart and brain were collected for pathological scores.The results were analyzed by SPSS 22.0 and GraphPad Prism 9.0 software and one-way analysis of variance(ANOVA)was used for comparison between groups.Results(1)At 12 h,the levels of AST(F=203.345,P<0.05),ALT(F=303.403,P<0.05),BUN(F=72.921,P<0.05),Cr(F=205.601,P<0.05),cTnI(F=35.991,P<0.05),CK-MB(F=42.815,P<0.05),amylase(F=1300.111,P<0.05),endotoxin(F=34.158,P<0.05),IL-6(F=541.858,P<0.05),TNF-α(F=191.687,P<0.05)in group B and C were significantly higher than those in group A;At 24 h,the levels of AST,ALT,BUN,Cr,cTnI,CK-MB,amylase,endotoxin,IL-6 and TNF-αin group B and C were significantly higher than those in group A.At 12 h,the levels of AST(189.33±23.47,65.83±6.08,F=155.732,P<0.05)and ALT(156.33±10.67,55.33±4.72,F=449.603,P<0.05)in group B were higher than those in group C,while the levels of BUN(8.80±0.56,11.02±1.33,F=14.131,P<0.05),Cr(35.30±2.65,58.77±3.65,F=162.136,P<0.05),cTnI(0.056±0.107,0.070±0.089,F=5.972,P<0.05),CK-MB(5.23±0.65,6.22±0.70,F=6.426,P<0.05),amylase(1445.89±70.37,2063.25±72.58,F=223.767,P<0.05),endotoxin(34.00±4.70,47.09±5.90,F=18.030,P<0.05),IL-6(102.18±7.01,
