摘　　要：目的研究乳腺区段切除术(PM)联合腋淋巴结清扫术(ALND)对早期乳腺癌的治疗效果及对血管细胞黏附分子-1(VCAM-1)水平的影响。方法乳腺癌患者110例随机分为对照组和观察组,每组55例。对照组接受乳腺癌根治术,观察组给予PM联合ALND术。观察2组患者手术一般情况,比较2组患者术后并发症发生率、手术后VCAM-1水平、生活质量和1年复发率、病死率的情况。结果观察组患者手术时间较对照组短,术中出血量较对照组少(P<0.05),2组患者清扫淋巴结数目差异无统计学意义(P>0.05);2组患者术后患侧上肢水肿、感觉异常和皮下积液发生率差异无统计学意义(P>0.05);2组患者术前、术后1 d和术后3 d的VCAM-1水平比较差异无统计学意义(P>0.05);观察组患者术后生活质量明显优于对照组,差异有统计学意义(P<0.05);2组患者术后复发率差异无统计学意义(P>0.05)。结论PM联合ALND治疗对早期乳腺癌患者的VCAM-1水平影响小,但能降低患者的术后并发症发生率,具有良好的应用价值。Objective To investigate the therapeutic effects of breast segmentectomy(PM)combined with axillary lymph node dissection(ALND)on early breast cancer and its effects on the levels of vascular cell adhesion molecule-1(VCAM-1).Methods A total of 110 patients with early breast cancer who were treated in our hospital from May to May 20,2017 were randomly divided into the control group and observation group,with 55 vases in each group.The patients in control group were treated by radical mastectomy,however,the patients in observation group were treated by PM combined with ALND.The general condition,and the incidence of postoperative complication,postoperative VCAM-1 levels,quality of life,1-year recurrence rate and mortality were observed and compared between the two groups.Results The operation time and intraoperative blood loss in observation group were significantly less than those in control group(P<0.05).There were no significant differences in the numbers of lymph nodes dissection and the incidence of upper extremity edema,paresthesia and subcutaneous effusion as well as the VCAM-1 levels before surgery,at 1d,3d after surgery between the two groups(P>0.05).Moreover the life quality of patients in observation group was superior to that in control group(P<0.05),however,there was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusion PM combined with ALND has little effect on VCAM-1 level in patients with early breast cancer,however,which can reduce the incidence of postoperative complications,with better application value.
