综合医院精神科门诊患者焦虑症状的网络分析

摘　　要：目的通过计算加权网络和预期影响探索综合医院精神科门诊患者焦虑症状网络的特征和核心症状。方法采用网络分析方法对8748例精神科门诊患者的焦虑症状进行分析,通过R软件进行统计分析和数据可视化。结果网络显示,症状“我因为一阵阵头晕而苦恼(A11)”和“我有晕倒发作,或觉得要晕倒似的(A12)”、“我手脚发抖打颤(A6)”和“我的手脚麻木和刺痛(A14)”之间具有最强的关联,其正则化偏相关系数分别为0.36和0.30。预期影响结果表明症状“我容易心里烦乱或觉得惊恐(A3)”的中心性程度最高,其预期影响为1.85(Z分数)。结论“我容易心里烦乱或觉得惊恐(A3)”是焦虑症状网络中最核心的症状。提示以该症状为核心靶点,针对性使用不同的干预措施可能会更大程度上降低精神科门诊患者整个焦虑症状网络的严重程度。Objective To explore the characteristics of anxiety symptom network and its core symptoms in psychiatric outpatients in general hospital by calculating weighted network and expected impact.Methods The anxiety symptoms of 8748 psychiatric outpatients were analyzed through network analysis method,and the R software was used for statistical analysis and data visualization.Results Two strongest edges existed between“I am distressed because of dizziness(A11)”and“I have a fainting episode or feel like fainting(A12)”(weight=0.36),between“My hands and feet are shaking and trembling(A6)”and“My hands and feet are numb and tingling(A14)”(weight=0.30).The expected impact results showed that the symptom“I am easily upset or panicked(A3)”had the highest centrality(Z-score=1.85).Conclusion“I am easily upset or panicked(A3)”is the core symptom in the anxiety symptom network.It reminds that targeted use of different interventions on this symptom may significantly reduce the severity of the overall anxiety symptom network in psychiatric outpatients.
