顺铂热化疗灌注联合肝动脉灌注化疗栓塞术治疗肝癌的远期疗效及对肿瘤细胞因子水平的影响

摘　　要：目的探究顺铂热化疗灌注联合肝动脉灌注化疗栓塞术(TACE)对肝癌患者远期疗效及肿瘤相关细胞因子的影响。方法选取2018年5月-2020年5月我院收治的80例肝癌患者作为研究对象,采用随机数字表法分为对照组和观察组,每组40例。对照组采用TACE治疗,观察组采用顺铂热化疗灌注联合TACE治疗,比较两组近期疗效、术后不良反应、肿瘤相关细胞因子指标(VEGF、HIF-2α、sIL-2R)、远期疗效。结果观察组客观缓解率为80.00%,高于对照组的57.50%,差异有统计学意义(P<0.05);两组肠胃道反应、骨髓抑制、皮疹、转氨酶升高的发生率比较,差异无统计学意义(P>0.05);观察组VEGF、HIF-2α、s IL-2R水平低于对照组,差异有统计学意义(P<0.05);随访1年后,观察组复发率低于对照组,总生存率高于对照组,无进展生存时间长于对照组,差异有统计学意义(P<0.05)。结论顺铂热化疗灌注联合TACE可提升肝癌患者的肿瘤控制效果,进一步降低肿瘤相关细胞因子水平的表达,且不增加不良反应发生几率,远期疗效理想。Objective To explore the effects of cisplatin thermochemotherapy combined with transcatheter arterial chemoembolization(TACE) on the long-term efficacy and tumor-related cytokines in patients with liver cancer.Methods A total of 80 patients with liver cancer admitted to our hospital from May 2018 to May 2020 were selected as subjects.They were divided into control group and observation group by random number table method,with 40 cases in each group.The control group was treated with TACE,and the observation group was treated with cisplatin thermochemotherapy perfusion combined with TACE.The short-term efficacy,postoperative adverse reactions,tumor related cytokine indexes(VEGF,HIF-2 α,s IL-2 R) and long-term efficacy were compared between the two groups.Results The objective remission rate of the observation group was 80.00%,which was higher than 57.50% of the control group,and the difference was statistically significant(P <0.05).There was no significant difference in the incidence of gastrointestinal tract reaction,bone marrow suppression,rash and elevated transaminase between the two groups(P>0.05).The levels of VEGF,HIF-2αand s IL-2 R in the observation group were lower than those in the control group,and the difference was statistically significant( P<0.05).After 1 year of follow-up,the recurrence rate of the observation group was lower than that of the control group,the overall survival rate was higher than that of the control group,and the progression-free survival time was longer than that of the control group,the difference was statistically significant( P<0.05).Conclusion Cisplatin thermochemotherapy perfusion combined with TACE can improve the tumor control effect of liver cancer patients,further reduce the expression of tumor related cytokines,and does not increase the probability of adverse reactions,and the long-term curative effect is ideal.
