摘 要:目的探讨外周血脂水平与原发性闭角型青光眼(PACG)之间的相关性,为青光眼的治疗提供临床依据。方法采用回顾性病例对照研究。选取2019年1月至2021年9月在郑州市第二人民医院眼科确诊的113例PACG住院患者作为病例组,选取同期健康管理中心的100例健康体检者作为对照组。采用全自动生化分析仪测定外周血胆固醇(CHO)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、脂蛋白a(Lpa)、载脂蛋白A-Ⅰ(ApoA-Ⅰ)、载脂蛋白B(ApoB)水平。比较两组血脂水平,采用多因素logistic回归分析影响PACG的相关因素。结果病例组CHO水平[(5.60±10.97)mmol·L^(-1)]高于对照组[(4.99±1.27)mmol·L^(-1)],差异有统计学意义(P<0.05)。病例组HDL-C水平[(1.14±0.26)mmol·L^(-1)]低于对照组[(1.33±0.37)mmol·L^(-1)],差异有统计学意义(P<0.05)。多因素logistic回归分析显示CHO水平与PACG的发病存在相关性(P=0.011)。结论PACG患者CHO水平较高,提示CHO水平升高是PACG发生的危险因素。临床中或许可以将外周血CHO水平作为预测PACG发生的一项生化指标。Objective To investigate the correlation between peripheral blood lipid level and primary angle-closure glaucoma(PACG),so as to provide clinical basis for the treatment of glaucoma.Methods A retrospective case-control study was conducted.A total of 113 inpatients with PACG who were diagnosed in Ophthalmology Department of Zhengzhou Second People’s Hospital from January 2019 to September 2021 were collected as case group,and 100 healthy physical examiners in health management center during the same period were collected as control group.The levels of peripheral blood cholesterol(CHO),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),lipoprotein A(Lpa),apolipoprotein A-Ⅰ(ApoA-Ⅰ),apolipoprotein B(ApoB)were determined by automatic biochemical analyzer.The blood lipid levels of the two groups were compared,and multivariate logistic regression was used to analyze the related factors affecting PACG.Results The level of CHO[(5.60±10.97)mmol·L^(-1)]in case group was higher than that[(4.99±1.27)mmol·L^(-1)]in control group,and the difference was statistically significant(P<0.05).The level of HDL-C[(1.14±0.26)mmol·L^(-1)]in case group was lower than that[(1.33±0.37)mmol·L^(-1)]in control group,and the difference was statistically significant(P<0.05).Multivariate logistic regression analysis showed that CHO level was correlated with the incidence of PACG(P=0.011).Conclusion The level of CHO in patients with PACG is high,suggesting that CHO level is a risk factor for PACG.In clinic,the level of CHO in peripheral blood may be used as a biochemical index to predict the occurrence of PACG.
