托吡酯片联合奥卡西平对成人部分性癫痫发作的疗效

摘　　要：目的探讨基于倾向性评分匹配托吡酯片联合奥卡西平在成人部分性癫痫发作(PS)治疗中的应用效果。方法回顾性分析2019年1月至2021年2月新乡市第一人民医院接受3个月的常规治疗和奥卡西平治疗的58例PS患者(对照组)资料,并收集同期接受3个月的常规治疗和奥卡西平联合托吡酯片治疗的62例PS患者(观察组)资料。用倾向性评分匹配分析患者基线资料,采用近邻法进行1∶1匹配,将观察组与对照组配对,经倾向性评分配比后,最终纳入观察组、对照组病例,各40例。评估两组治疗3个月时临床疗效;对比两组治疗前、治疗3个月时的脑电图变化(监测α、β、θ及δ频段的相对功率);记录两组治疗期间不良反应发生情况。结果经倾向性评分匹配后,两组性别、年龄、体质量指数(BMI)、病程、吸烟史及饮酒史比较,差异无统计学意义(P>0.05);治疗3个月时,观察组治疗总有效率高于对照组(P<0.05);治疗3个月时,两组的α、β、δ、θ频段的相对功率均较治疗前降低,且观察组低于对照组(P<0.05);两组的不良反应发生率对比,差异无统计学意义(P>0.05)。结论托吡酯片联合奥卡西平可更加有效提高成人PS患者的治疗效果,促进脑电波恢复稳定,且安全性好。Objective To explore and analyze the application efct of topiramate tablet combined with oxcarbazepine in the treatment of partial seizures(PS)in adult based on propensity score matching.Methods The data of 58 patients(the control group)with PS who received routine treatment and oxcarbazepine treatment for 3 months from January 2019 to February 2021 in Xinxiang First People’s Hospital were retrospectively analyzed.The data of 62 patients(the observation group)with PS who received routine treatment and oxcarbazepine combined with topiramate tablet for 3 months in the same period were collected.Baseline data of patients were analyzed by propensity score matching,and the nearest neighbor method was used for 1:1 matching,and the observation group was paired with the control group.After propensity score matching,40 cases in the observation group and 40 cases in the control group were eventually included.Clinical efficacy of the two groups was evaluated after 3 months of treatment.The EEG changes of both groups before and 3 months after treatment were compared(monitoring the relative power of α,β,θandδfrequency bands).The adverse reactions of the two groups during treatment were recorded..Results After propensity score matching,there was no statistical significant difference in gender,age,body mass index(BMI),course of disease,smoking history and drinking history between two groups(P>0.05).Compared with the control group,the total effective rate of the observation group after 3 months of treatment was higher(P<0.05).Afer 3 months of treatment,the relative power of a,β,θandδfrequency bands in both groups were decreased than that before treatment,and compared with the control group,the observation group was lower(P<0.05).Compared the incidence of adverse reactions between the two groups,there was no stat istical significant difference(P>0.05).Conclusion Topiramate tablet combined with oxcarbazepine can more effectively improve the therapeutic effect of adult PS patients and promnote the recovery and stability of
