稳定型咬合板联合玻璃酸钠关节注射法治疗青少年颞下颌关节不可复性盘前移位的疗效分析

摘　　要：目的:探讨研究用稳定型咬合板联合玻璃酸钠关节注射法治疗青少年颞下颌关节不可复性盘前移位的疗效。方法:选取2020年7月至2021年7月克拉玛依市中心医院收治的颞下颌关节不可复性盘前移位患者56例,将其随机分为试验组与参照组(28例/组)。对参照组患者进行常规的稳定型咬合板治疗,在此基础上对试验组患者进行玻璃酸钠关节注射治疗,然后比较两组患者颞下颌关节的功能障碍指数(DI)、肌肉触痛指数(PI)和下颌运动功能指标(包括无痛最大开口度(MVO)、下颌前伸距离(PE)、左侧偏动距离(LDD)和右侧偏动距离(VDD))。结果:(1)治疗前,两组患者颞下颌关节的DI和PI相比,差异无统计学意义(P>0.05)。治疗后,试验组患者颞下颌关节的DI和PI均低于参照组患者,差异有统计学意义(P<0.05)。(2)治疗前,两组患者的MVO、PE、LDD、VDD相比,差异无统计学意义(P>0.05)。治疗后,试验组患者的MVO、PE、LDD、VDD均大于参照组患者,差异有统计学意义(P<0.05)。结论:用稳定型咬合板联合玻璃酸钠关节注射法治疗青少年颞下颌关节不可复性盘前移位可取得较为理想的疗效。Objective:To investigate the efficacy of stable occlusal plate combined with sodium hyaluronate joint injection in the treatment of adolescent temporomandibular joint with irreversible anterior disc displacement.Methods:A total of 56 patients with irreversible anterior disc displacement of TEMPORomandibular joint admitted to Karamay Central Hospital from July 2020 to July 2021 were selected and randomly divided into experimental group and reference group(28 cases/group).The control group was treated with conventional stable occlusal plate,and the experimental group was treated with sodium hyaluronate joint injection on this basis.Then,temporomandibular joint dysfunction index(DI),muscle tenderness index(PI)and mandibular motor function indexes(including painless maximum opening(MVO),mandibular extension distance(PE),left deviation distance(LDD)and right deviation distance(VDD))were compared between 2 groups.Results:(1)Before treatment,there was no statistical significance in THE DI and PI of TMJ between 2 groups(P>0.05).After treatment,the DI and PI of the TEMPORomandibular joint in the experimental group were lower than those in the reference group,with statistical significance(P<0.05).(2)Before treatment,there were no significant differences in MVO,PE,LDD and VDD between 2 groups(P>0.05).After treatment,the MVO,PE,LDD and VDD of the experimental group were higher than those of the reference group,with statistical significance(P<0.05).Conclusion:Stable occlusal plate combined with sodium hyaluronate joint injection can achieve ideal results in the treatment of adolescent TMJ.
